Objective:Plasma miR-21 levels were measured in patients with acute aortic dissection(AAD).Evaluate the diagnostic value of miR-21 to AAD by using the ROC curve and related clinical data.Methods:(1)Subjects: 33 patients with acute aortic dissection were collected from the Department of cardiac surgery,Sun Yat-Sen Cardiovascular Hospital,Shenzhen as experimental group(Group A),30 patients with angina pectoris(Group B),acute myocardial In 29 patients with infarction(Group C),There were no family history of cardiovascular disease,and no previous chest pain performance.29 volunteers who were excluded from cardiovascular and tumor-related diseases by physical examination were control group(Group D).The clinical data of each group were also collected.(2)Collection and preservation of plasma samples: In each group of patients,peripheral venous blood was collected after diagnosis,plasma was obtained by centrifugation within 15 minutes,and stored in a refrigerator at-80°C until use.(3)Detection of miRNAs expression levels: qRT-PCR was performed to detect the expression levels of plasma miR-21 and internal reference gene miRNA-16,and the relative expression levels of miRNA-21 were calculated.(4)Statistical Analysis: SPSS 17.0 statistical software was used to analysis the data.The diagnostic reliability of plasma miR-21 in AAD was evaluated using the receiver operating curve(ROC curve)and the area under the curve.AUC at 0.5-0.7,the diagnostic value is low,AUC at 0.7-0.9,the diagnostic value is general,AUC>0.9 has higher diagnostic value.Results:(1)Compared with the normal control group,the expression level of plasma miRNA-21 was significantly higher in patients with AAD([0.05(0.03,0.12)] vs [0.04(0.04,0.05)],p<0.05).(2)Compared with plasma samples from patients with angina pector is,and acute myocardial infarction patients,there was no significant difference in plasma miRNA-21 expression between patients with AAD(p>0.05).(3)There was no significant difference in plasma miRNA-21 expression between patients with a history of hypertension in the AAD group and those with a negative history of hypertension([0.05(0.03,0.10)] vs [0.09(0.02,0.16)],P >0.05).(4)ROC curve analysis showed that the area under the curve(AUC)was 0.610,95% confidence interval was 0.464~0.756,when the cutoff value was 0.359,the sensitivity of plasma miR-21 diagnosis of aortic dissection was 69.7%.The specificity is 24.1%.Conclusions:The expression of plasma miR-21 in patients with AAD is increased,and the difference is significant.The detection of plasma miR-21 has certain clinical value in the diagnosis of AAD,but lacks specificity.This study is not enough to support its as an ideal molecular diagnostic marker for AAD.
