[Background]Burns is an unexpected accidental injury which is common and frequent in outpatient and emergency departments. It not only causes more pain in the process of wound healing, but also brings about serious psychological burden on the patients, causing them to worry about the appearance and function of the wound after healing. Thus, for burn doctors, how to correctly treat burn wounds is the most critical issue at present. In addition to preventing wound infection and further aggravation,it is also necessary to reduce the patient's pain during the dressing changing and reduce the exudation and edema of the wound. Therefore, finding a suitable biological dressing is particularly important for the early repair of burn wounds, which not only effectively prevents wound infections and reduces pain, but also provides a moist, hypoxic environment for skin and promotes the formation of skin attachments during epithelialization.Burn wounds are a key point in the treatment of burns. For decades, medical scientists around the world have been actively promoting the rapid repair of burn wounds through various therapies. With the in-depth study of wounds by scientists and the medical field in recent decades, there are plenty of breakthrough progresses have been made in burn dressing. It is not only realized that the wound can not be covered simply, but also need proper dressing to promote wound healing and prevent infection effectively.Advances in biology, technology, and materials science have enabled the rapid development of various types of wound dressings, such as heterogeneous skin, composite application dressings,or dressings containing gels and drugs, which are currently hot issues in research and application. With the continuous development and advancement of society and medicine, the clinical requirements for burn dressings will be increasingly higher. It is universally acknowledged that early active and effective wound closure is one of the most critical and most effective measures, promoting the rapid healing of mild burns and significantly improving the survival rate of patients with deep burns in the middle and large areas.Although there is a certain distance between the requirements of various biological burn dressings and ideal burn dressings,it is sure that in the near future, satisfactory and high-quality biological dressings will come into being to benefit the people around the world.[Objectives]Analyze the value of artificial dermal substitutes for repairing refractory wounds, and make recommendations for clinical research.[Methods]Randomly selected five patients with various refractory wounds treated in our hospital from October 2017 to April 2018.The patients were thoroughly debrided with a single operation.Artificial dermis was transplanted.Local granulation tissue grew well after 3 weeks.Extensive tendons or bone tissue are covered by dermal tissue. Then the autograft was transplanted in the second stage to observe the survival rate of the graft after artificial dermis replacement. At the same time, the hyperplasia of the scar was observed. The joint activity and pigmentation of the functional part of the patient were evaluated synthetically with the autologous skin.Transplantation of tablets to repair refractory wounds.[Clinical Experience]:Compared to traditional surgical procedures, the number of operations and hospitalization are significantly reduced, the autologous tissue donor site is minimally injurious, mild scar hyperplasia,no rejection, and good postoperative appearance.Functional exercise can be performed early.[Research result]Artificial dermis substitutes have positive significance for the repair of refractory wounds, which can reduce the degree of scar hyperplasia, help patients recover body functions, and have certain reference value for clinical treatment.
