The Substructure Research About Preventing Sustained Cutaneous Burns Following Sulfuric Acid Burn

Sulfuric acid burn is one of the frequent surgical diseases. It always causes severe injury to expose skin. If the burn surface can't be treated well in time, it will form terrible sequella following the burn surface heal. And it must leave large pain on patients' body and soul, and degrade their quality of life. At present, the ways about therapy are covering the crust with some drugs like 1% silver sulfadiazine cream or something else, and then doing surgical operation to clear necrosis tissue and graft healthy skin to cover the burn surface. But we don't know how the burn tissue changes and which time is the best for operation. So in our experiment, by burning skin of pigs with different concentration of sulfuric acid, we want to find how the different surfaces changes when they are treated by different therapies, and also we hope to find the best time for operation.[Objectives] To identify the early managements about sulfuric acid burn, observe the relationship between extent of necrosis tissue and concentration of sulfuric acid. To find which the best time of operation is.[Methods] To burn both sides of dorsal ridges of pigs by different concentrations of sulfuric acid including 50 percent, 70 percent, 95 percent and 99 percent. The left sides were control group and the right sides were experiment group. The control group was sulfuric acid burn surfaces without any managements, the experiment group was sulfuric acid surfaces with the therapy of irrigating substantial volumes of water and last 30 minutes. Then at the times of 0min, 15min, 30min, 1h, 2h, 4h, 8h, 12h, 24h, get tissue from the burn surface and by the means of Masson's staining to find out the changes of the burn surface in future time burned by the sulfuric acid. We also record the development of the burn surface and the time when the surface heal to find out the time when the sulfuric stop permeating the tissue. To give surgical operation before this time will be the best. In the future time, we need to record the days which each burn surface needs to heal, so we will identify the role of the water. Meanwhile through comparing the depth we observed by the microscope, we can look forward to the best time of giving the operation to different burns.[Results] Different concentration sulfuric acids cause different burns, the concentrated of 99 percent and 95 percent can produce deep cutaneous burns and the concentrated of 70 percent and 50 percent cause less serious burns. Every cutaneous burn are becoming more and more serious, the deep cutaneous burns stop in about 2 hours while the light cutaneous burns stop in about 4 hours. Irrigation of the burn site promptly with substantial volumes of water and an adequately long time will help largely reduce the morbidity from sulfuric acid burns. By the way, when we irrigated the burn surface following that, we found the leaving sulfuric acid burn healthy skin with the water. So we should emphasize the importance of large volumes of water, also we can use cloth to displace the odd sulfuric acid before wash it. It will be receive better effect.[Conclusions] Different cutaneous burns caused by different concentrations of sulfuric acid have different ways in their changes. Irrigation of the burn site promptly with substantial volumes of water in time and an adequately long time will help largely reduce the morbidity and the extent of sulfuric acid burn. The best operation time about sulfuric acid burns is in 4 hours, the earlier the better.
