This study aims to investigate the clinical significance and clinical significance of sST2 in the diagnosis of heart failure and to find outthe effect of different levels of renal function on the diagnosis of heart failure using sST2,providing new ideas for the diagnosis and treatment of heart failure.According to the NYHA classification,73 patients with heart failure with or without renal dysfunction who wer ehospitalized in Department of Cardiology of Hebei Provincial People’s Hospital were selected as the experimental group.The patients were divided into three groups according to glomerular filtration rate(eGFR)Were further divided into 22 cases of normal renal failure group,22 cases of mild renal dysfunction group,and 29 cases of moderate a nd severe renal insufficiency group.In the same period of hospitalization of 15 cases of purely hypertensive,arrhythmia patients as acontrol group.On the basis of routine examination,the serum levelsof sST2,e GFR,hs-CRP and BNP in all the patients were tested and echo cardiography was performed and collected.Therefore,the generalclinical data of patients were included.SPSS21.0 software was used for statistical analysis of the data,using two-sided test,Pvalue<0.05 that the difference was statistically significant.Results showed that.1 There was no significant difference in the general clinical data between the experimental group and the control group(P>0.05),labor atorytest results,blood tests,biochemical indicators and other laboratory tests were not statistically significant Difference(P>0.05).The results of echocardiography showed that the left ventricular ejection fraction(LVEF),left atrium(LA),left ventricular posterior wall thickness(LVPWT),leftventricular end systolic diameter(LVESD)and left ventric ular end diastolic diameter There was a significant difference(P<0.01).2 Serum BNP,sST2 levels in the experimental group and the control groupwere significantly different(P<0.05).3 There was no significant difference in serum sST2 levels between different renal function groups inpatients with heart failure(P>0.05),and there was significant difference in BNP levels between different renal functional groups(P<0.05)With the severity of renal function increased significantly increased.4 Spearman correlation analysis: There was a positive correlation between serum sST2 and BNP levels and NYHA classification(r=0.319,0.479,P<0.05),and a negative correlation with LVEF(r=-0.311,-0.426,P<0.05).5 Comparison of serum sST2 and BNP in diagnosis of heart failure ROC curve analysis showed that the area under curve(AUC)of serum BNP and sST2 were 0.973,0.751(P<0.05),the cut-offpointswere68.350,198.510 pg / m L,the sensitivity was 97.3%,79.5%,specificity were 93.3%,73.3%.Our conclusion is that,1.sST2 level canreflect the severity of heart failure patients,is an effective indicatorofhear tfailure,heart failure can be used as a biomarker.2.sST2,BNP and the diagnosis of patients with chronic heart failure is closelyrelated to BNP serum concentration with the severity of renal insufficiencychan g-es,however,sST2 is not affected by renal function,can better reflectthe severity of the patient’s condition,Heart failure diagnosisofpatien tswith some value.3.The level of sST2 was positively correlated witht he severity of BNP and heart function,and negatively correlated with LVEF.The detection of sST2 level could be used as an important complement to BNP echocardiography to increase the accuracy of di-agnosis and differential diagnosis of heart failure.
