Purposes:The purpose of this study was to explore the effect of pain and muscle mechanical function on patients with patellofemoral pain syndrome.by acupuncturing quadriceps trigger points with isokinetic testing of the knee at 60°/s relative peak torque, average power and total work. Methods:Patients with patellofemoral pain syndrome(n=50)were randomly divided into two groups from the Shanghai University of Sports Rehabilitation Center Pain Clinic,Hudong Hospital Pain Clinic(Shanghai)and volunteers who participated in this trial.Group(n=25)and control(n=23).Acupuncturing quadriceps trigger points combined with posterior quadriceps stretching(once a week, for 6 weeks)was performed in both groups. The control group was sham acupuncture(the needle did not penetrate the skin, only Leather).Pain scores(VAS;0-10)and knee injury scales(KOOS;0-100points)were used to assess knee pain and function. The time points were assessed before, after, and after 3 months of follow-up.Before and after the clinical treatment, the patients were subjected to isokinetic tests of 60°/s.The evaluation indexes were60°/s peak torque, average power and total work.Two-factor repeated measures analysis of variance was applied to statistical analysis. Results:Pain:Within the group comparison, experiment group and the control group showed that there was no significant difference in the VAS scores before and after treatment and after 3 months of follow-up in the control group(P>0.05);VAS scores in the experiment group were significantly lower than those before treatment(P<0.01)during the mid-treatment,after treatment, and after 3 months of follow-up.Between groups, there was no significant difference between the control group and the experimental group in the mid-treatment period.Compared with the control group, the VAS scores of the experimental group were significantly lower after 6 treatments and after 3 months(P<0.01).Within the group comparison, experiment group and the control group showed that there was no significant difference in the KOOS scores before and after treatment and after 3 months of follow-up in the control group(P>0.05);before and after treatment and in March in the experiment group After the follow-up,the KOOS score wassignificantly lower(P<0.01);there was no significant difference between the KOOS score and the follow-up after 3 months in the experimental group.Between groups, compared with the control group, the KOOS score was significantly lower in the experimental group and after the 3 months follow-up(P<0.01).Muscle function:Statistical analysis of the of 60°/s showed that there was no significant difference in the extensor and flexor peak torque, average power and total work after treatment between the control group and the experiment group(P>0.05).The extensor and flexor peak torque, average power and total work of the experiment group after treatment were significantly higher than before treatment(P<0.01).Between groups, peak torque, average power and total work of extensor and flexor muscle in the experimental group were significantly higher than those in the control group(P<0.01).The statistical analysis of the ratio of the flexion and extension muscle peak moments showed that the ratio of the flexion and extensor peak torque after treatment in the control group was not significantly different from that before treatment(P>0.05);There was no significant difference in the ratio of the peak torque of the flexors and extensors after the treatment(P>0.05).Between groups, there was no significant difference in the ratio of flexion and extensor peak torque after treatment between the experimental group and the control group(P>0.05).Conclusion:On the one hand, this study analyzed the subjective index VAS and KOOS scores and analyzed objective indicators of knee isokinetic at 60°/s relative peak torque, average power and total work indicates that acupuncturing quadriceps trigger points can reduce patellofemoral pain in the patient and the muscle function of the quadriceps and hamstring muscles is improved.
