objective The purpose of this study is to summarize and track the follow-up results of BI-RADS 3 types of breast lesions,the value of follow-up and the sonographic features that may be valuable for diagnosing breast malignant lesions and basic data were analyzed,so as to lay the foundation for further clinical treatment.Methods The breast lesions found in the ultrasound department of Jiangnan University Affiliated Hospital(the Fourth People's Hospital of Wuxi)from June 2014 to February 2018 were reviewed in this study,BI-RADS 3 breast lesions assessed by ultrasound were selected.The breast lesions with follow-up results were summarized and the reasons for the escalation and demotion of the breast lesions were analyzed.At the same time,the pathologically confirmed breast lesions were statistically analyzed,and the pathological types and image features of the malignant lesions were analyzed.The data that may affected benign and malignant breast lesions were collected,including age,family history,BMI,history of smoking,accessible or not,the size of the lesions,and evaluate the types of breast mammography.Results In this study,3132 patients who were initially assessed as BI-RADS 3 breast lesions by ultrasound were collected.After the follow-up,there were 190 cases of BI-RADS classification upgrade,1078 cases of BI-RADS classification reduction,and1864 cases of BI-RADS classification.The reasons for the upgrade include irregular shape,enlarged volume,irregular edges,high echo,halo,posterior echo attenuation,catheter dilation and microcalcification.The reasons for the degradation include the size of the breast lesion,the multiple lesions of the bilateral breast,the complex cysts,the size of the lesion and the disappearance of the lesion.Of all the 3132 patients,489 patients had pathological results,of which 468 were benign,21 were malignant,and the incidence of malignant lesions was 4.3%.The majority of malignancies were ductal carcinoma in situ(9 cases),followed by invasive ductal carcinoma(4 cases),papillary carcinoma(6 cases)and mucous adenocarcinoma(2 cases).BI-RADS upgraded 190 cases of breast lesions,18 cases of breast malignant lesions,the incidence of malignant lesions was 9.5%,BI-RADS classification remained 3 cases of breast lesions in 299 cases,of which 3 cases were malignant lesions,and the incidence of malignancy was 1%.In the ultrasonic images of malignant breast lesions,the edge of the breast lesions is irregular and the number of catheter dilatation is in the majority.There are great significance in age,family history and breast type on the reevaluation of malignant and benign breast lesions(P < 0.05),while BMI,smoking or drinking,accessible or not,the sizes of lesions for 3 BI-RADS ultrasound initial evaluation of benign and malignant breast lesions had no significant difference(P > 0.05).Conclusion After follow-up,it was found that ultrasound was first evaluated for BI-RADS 3 types of breast lesions and should follow the guidelines for the treatment of breast diseases.At the same time,a comprehensive assessment and analysis can be made according to the specific conditions of the patient,including age,family history,and breast type.In order to avoid unnecessary biopsy or short interval follow-up,further prospective studies on the annual follow-up of BI-RADS 3 types of breast lesions are also necessary.
