Study On The Correlation Of Endogenous Carbon Monoxide In Patients With Acute Exacerbation Of Chronic Obstructive Pulmonary Disease

Objective:By evaluating the carbon monoxide content in the arterial blood of patients with AECOPD in our hospital,analyze its relationship with lung function,Pa O2 and PCO2,to provide a basis for accurate assessment of the patient’s condition.Methods:34 of adult COPD patients with acute exacerbation hospitalized in our hospital respiratory department from January 2017 to February 2017 were selected as experimental group.At the same time,26 cases of healthy people were selected as control group from the hospital physical examination center.Arterial blood gas were detected in all individuals and lung function were also measured.Then,the carbon monoxide content in the arterial blood were compared between the experimental group and the control group,and according to the 2017 GOLD,the experimental group were divided into Ⅱ level(moderate group)(8 cases),Ⅲ(severe group)(15 cases),Ⅳ(extreme severe group)(11 cases).At the same time,according to the level of Pa O2 and PCO2,the experimental group were divided into respiratory failure group(11 cases)and normal group(23 cases).The endogenous carbon monoxide levels were compared between this two groups.Results:1.There were no statistically significant difference between the experimental group and the healthy body in age and gender(P >0 0.05).2.The level of arterial blood COHb(4.41 ± 1.373)in the experimental group was significantly higher than that in the control group(3 ± 1.3).There were statistically significant differences between the two groups.3.The arterial blood COHb levels of the experimental group in the moderate group(8 cases),severe group(15 cases)and extreme severe group)(11 cases)were(3.5 + 1.195)%,(4.13 + 1.125)%,(5.45 + 1.214)%.There were statistically significant differences in this three groups(F = 7.219,P = 0.003 < 0.01).By the above result,as the decline of FEV1%pred,the concentration of COHb in arterial blood were increased.Using correlation analysis,the levels of COHb in arterial blood and the date of FEV1%pred were negative correlation.4.The levels of COHb in arterial blood between the patients with respiratory failure and normal group in the experimental group were(5.27 + 1.104)% and(4 + 1.314)%.There were statistically significant difference between the two groups(P < 0.01).Conclusions:1.The levels of COHb in arterial blood in patients with AECOPD was significantly higher than healthy people and it was seriously related to the disease.2.The levels of COHb in arterial blood in patients with AECOPD was negatively correlated with the FEV1%pred.3.The levels of COHb in arterial blood in patients with AECOPD was related to the degree of hypoxia.
