Objective:To investigate the cardiopulmonary exercise tolerance and to study the impact factors for blood pressure and cardiopulmonary endurance of patients with essential hypertension.Methods:100 cases were selected according to the criteria of the subjects:(1)According to the resting blood pressure level of patients with essential hypertension,they were divided into three groups: group 1(systolic blood pressure?159mm Hg and/or diastolic blood pressure?99mm Hg)(n=47),group 2(160mm Hg?systolic blood pressure?179mm Hg and/or 99 mm Hg?diastolic blood pressure ?109mm Hg)(n=33)and group 3(systolic blood pressure?180mm Hg and/or diastolic blood pressure?110mm Hg)(n=20).The cardiopulmonary function tester of our cardiovascular disease rehabilitation center was used to monitor heart rate,heart rhythm and ECG changes.A uniform table was designed to record the maximal oxygen uptake(VO2 max/kg),exercise time,maximum exercise load,maximum heart rate,and maximum bloodpressure per kilogram of body weight during the three exercise tests,and to calculate the corresponding metabolic equivalents(METs).We also evaluated cardiopulmonary exercise tolerance levels of different group.At the same time,1 minute,3 minutes,5 minutes and 15 minutes of heart rate and blood pressure were recorded during the exercise recovery to calculate the short-term variability of blood pressure and heart rate during exercise recovery.(2)According to occurrence of exercise hypertension during exercise,the subjects were divided into exercise hypertension group(n=43)and non-exercise hypertension group(n=57),and their resting blood pressure was recorded.The arteriosclerosis tester and visceral fat tester were used to examine the brachial pulse wave conduction velocity(ba PWV),ankle brachial index(ABI)and visceral fat index in each group.(3)According to the level of VO2 max/kg(ml/kg·min)of the subjects,referring to the Weber-Janicki criteria,patients were divided into group 1(VO2 max/kg ? 20)(n=14),group2(16?VO2max/kg<20)(n=23),group 3(10?VO2max/kg<16)(n=57)and group 4(VO2max/kg<10)(n=6).The correlation between VO2 max/kg and short-term blood pressure variability,visceral adiposity,arteriosclerosis,neck circumference,hip circumference and abdominal circumference during exercise recovery were compared.Results:1.There was a significant difference in the METs,maximum exercise load,exercise time,and short-term blood pressure variability during exercise recovery in the hypertension group 1,group 2 and level3 groups.With the increase of resting blood pressure,METs value,maximum exercise load and exercise time were significantly reduced.2.The resting blood pressure level,short-term blood pressure variability,ba PWV,ABI,BMI,and visceral fat levels in the exercise hypertension group were significantly higher than those in the non-exercise hypertension group.3.With the decrease of VO2max/kg,the levels of abdominal circumference,hip circumference,visceral fat,and bilateral ankle-brachial index were significantly increased and the difference was significantly different,and VO2max/kg was significantly associated with visceral fat.Conclusion:1.There are significant differences in heart rate and short-term BPV during exercise and exercise recovery in patients with essential hypertension who have different resting blood pressure levels.It issuggested that attention should be paid to the changes of heart rate and blood pressure during the recovery of exercise and exercise in patients with essential hypertension.CEPT test results are the exact basis for the formulation of exercise prescriptions for hypertensive patients.2.With the increase of resting blood pressure,arteriosclerosis,visceral fat and BMI in patients with essential hypertension,exercise hypertension is more likely to occur.As the occurrence of exercise-induced hypertension in patients with elevated blood pressure during exercise,cardiovascular and other adverse events are likely to occur,so some patients need to be CPET examination to guide their safe and moderate exercise.3.There is a negative correlation between resting blood pressure and exercise tolerance in patients with essential hypertension.Because exercise tolerance is significantly associated with the prognosis of patients with cardiovascular disease,blood pressure control in patients with essential hypertension benefits more at a more stringent level.4.The visceral fat in patients with essential hypertension is inversely related to the level of cardiorespiratory endurance.Patients with visceral fat excess need CPET examinations to guide them to perform sub-average,individualized exercise and avoid excessivecardiovascular damage due to prolonged excessive exercise.
