Analysis Of Changes Of Etiology For Ten Years And Clinical Characteristics Of Elderly Patients With Chronic Kidney Disease

Objective:By retrospectively analyzing the age,sex,etiology,and related clinical data of elderly inpatients with chronic kidney disease(CKD),to clarify the etiological changes and clinical characteristics of elderly CKD in the past decade,aiming to provide a reliable theoretical basis for the early recognition,early prevention and early treatment of diseases.Method:In this study,we investigated medical records of the patients in nephrology department from the electronic database in the First Hospital of Jilin University.We investigated inpatients in different years of 2006,2011 and 2016.The patients were grouped according to the year of hospitalization and the disease characteristics,etiology structure and first symptoms were statistically analyzed by SPSS 22.0 statistical software.Results:1.We analyzed the data of 679 elderly inpatients with CKD: the proportion of male in elderly CKD inpatients respectively accounted for 35.6%,43.3%,47.8% in years of 2006,2011 and 2016;the female account for 64.4%,56.7%,and 52.2%.The average age of the elderly inpatients in the study group was 71.16±4.95,72.48±5.96 and 72.16±6.06,respectively.The inpatients in each year were classified by age: 65-70,71-75,76-80,81-85 and more than 86 classification.A high incidence rate of kidney disease occurred at the age of 65-70 years in three years,respectively account for 49.0%?42.8%?50.3%.Analysis of complications: Hypertension ratio: 36.2%,55.8%,79.8%;Diabetes: 19.5%,36.1%,41.9%,showing an increasing trend year by year.2.The etiology constitution ratio of elderly inpatients with CKD: The main reasons in 2006 were drug-induced kidneyinjury,followed by hypertensive nephropathy,chronic pyelonephritis,and chronic glomerulonephritis;diabetic nephropathy was the primary cause in 2011 and 2016,followed by hypertension nephropathy,chronic glomerulonephritis,and drug-induced kidneyinjury.The proportion of drug-induced kidneyinjury was 27.5%,13.9% and 6.2%,and the difference of the cause of drug induced renal damage in 3 years was statistically significant(c2=40.338,P<0.001),showing a downward trend.The proportion of diabetic nephropathy in years of 2006,2011 and 2016 was 14.8%,28.4% and 24.5% respectively.The difference of the cause of diabetic nephropathy was statistically significant(c2=9.235,P=0.010),showing an upward trend.The proportion of hypertensive nephropathy in three years was 18.8%,26.9%,and 18.6%,respectively,accounting for the second cause of the disease,there was no statistical difference in the causes of hypertensive nephropathy among the three years(c2=5.863,P=0.053).The proportion of chronic pyelonephritis was 17.4%,3.8% and 2.5% respectively.The difference of etiology of chronic pyelonephritis in 3 years was statistically significant(c2=42.141,P<0.001),showing a downward trend.The 2006 and 2011 years were dominated by Guanxin Suhe pills,followed by pain drugs;in 2016,analgesics were mainly used as painkillers,followed by Guanxin Suhe pills;the proportion of Guanxin Suhe pills was53.7%,37.9% and 15%,respectively,showing a downtrend.3.The first symptoms of CKD inpatients in 2006,2011,and 2016 were mainly edema;followed by nausea,vomiting,anorexia andfatigue.The three annual symptoms of nausea,vomiting,anorexia andfatigue showed a downward trend.4.Analysis of renal biopsy in CKD inpatients: 0 case in 2006;10 cases in 2011;27 cases in 2016.The pathological results of kidney biopsy in 2011 and 2016 were mainly membranous nephropathy,accounting for 40% and 55.6% respectively.Conclusions:1.Elderly CKD hospitalized inpatients are mostly women,mainly 65-70 years old.2.Diabetic nephropathy has become the main cause of elderly CKD inpatients in recent years,followed by hypertensive nephropathy,chronic glomerulonephritis,and the proportion of chronic interstitial nephritis is decreasing.3.The first symptom of elderly CKD inpatients was mainly edema,common in both lower extremities,eyelid edema.4.The number of renal biopsy patients in elderly inpatients with CKD is increasing,and pathological results were mainly membranous nephropathy.
