Objectives: To investigate the effect of smoking status on in-hospital outcome of acute ST-segment elevation myocardial infarction treated with primary percutaneous coronary intervention. Methods: This analysis included 1432 patients admitted with acute STEMI undergoing primary PCI at the department of cardiology of the first Bethune Hospital,Jilin University from January 2014 to November 2016,and they were divided into non-smokers,current smokers,ex-smokers groups according to smoking status.According to the inpatient's medical records,we collected the basic information of all the patients, the risk factors of coronary heart disease, the past medical history, the Killip grade of admission, the results of laboratory examination, coronary angiography, adverse cardiovascular events in hospital and so on.The above data are analyzed. Results: A total of 1432 patients were included in the study.Current smokers were more likely to be younger(p<0.05),male(p<0.05)and have fewer comorbidities, including hypertension ? Diabetes and so on.Current smokers had a greater proportion of single-vessel disease, and the Gensini score was lower than the non-smokers and the ex-smokers.There was no difference between groups on all cause mortality(p = 0.057)and MACEs(p =0.777).In multivariate analysis Gensini scores is correlate with all cause mortality. Conclusions: Given the apparent advantageous demographics in current smokers, namely, younger age, greater preponderance of males, with fewer comorbidities, the degree of vascular lesion is relatively mild. There is no survival advantage in patients admitted with acute STEMI undergoing primary PCI.Gensini score is an independent risk factor for in-hospital mortality in acute STEMI patients undergoing PCI.
