Objective:Comparative analysis of the application of transabdominal and laparoscopic surgery in the treatment of early malignant ovarian tumors.Methods:A retrospective analysis of 1423 patients with ovarian cancer admitted from January 1,2010to December 31,2014 in the Tumor Hospital Affiliated Xinjiang Medical University was performed.Clinical data of 155 cases of early epithelial ovarian cancer diagnosed by pathological diagnosis of FIGO stage IA-IIB Acording the operative methods were divided into laparoscopy group(n=47)and transabdominal group(n=108).The patients in both groups were evaluated as satisfactory tumor cell reduction operation,and the planned chemotherapy was completed after operation.The operative time,the amount of blood loss,the total number of lymph nodes,the recovery time of gastrointestinal function,time to first course of chemotherapy after operation and the postoperative complications were compared between the laparoscopic group and the transabdominal group.After 3 years follow-up,the recurrence rate of two groups of patients with different surgical methods was observed.Retrospective analysis of 3-year survival rate and 3-year survival rate.Results:Compared with the transabdominal group,the amount of blood loss in the laparoscopic group was small[175.95±80.06ml vs.292.55±160.37ml,t=-3.807,P=0.000],and the recovery time of the gastrointestinal function was short[1.62±0.54d]Vs3.47±1.41d,t=20.377,P=0.000].There was no significant difference in the number of intraoperative lymph node dissections[23.21±12.57 vs.21.83±13.28,t=0.956,P=0.187].The tumor was removed during operation.Smaller size[6.60±4.99vs.8.95±6.15,t=6.13,P=0.001],Short postoperative hospital stay[11.23±5.6d vs.15.53±7.7d,t=-3.566,P=0.004],The time from first course of chemotherapy after operation was short[8.98±3.086d vs.19.75±8.698d,t=-14.296,P=0.000],and the postoperative complication rate was low[6.38%vs.17.59%,?~2=10.456,P=0.001],there was no significant difference in operation time[212.39±71.26min vs.222.35±65.33min,t=1.175,P=0.082].There was no significant difference in the recurrence rate after 3 years[29.8%vs.31.5%,?~2=0.029,P=0.864],there was no significant difference in postoperative 3-year survival rate[73.17%vs.70.79%,?~2=0.005,P=0.944].Conclusion:The application of laparoscopy in early ovarian malignancy has the advantages of less trauma,less bleeding,faster postoperative recovery,fewer postoperative complications,and less abdominal wall incision.Compared with the transabdominal group,it does not increase the recurrence rate of patients after 3 years of operation and Mortality and other advantages.Laparoscopy provides a feasible approach for staging of early stage ovarian cancer.
