Objective:To explore the efficacy and safety of ERCP in patients with biliary complications of hepatic echinococcosis.Methods:Retrospective analysis of our hospital from September 2014 to December 2017 liver hydatid to break into the 32 cases of biliary tract,according to whether the 32 patients preoperative ERCP application(ERCP)in treatment is divided into the following two groups.Applying preoperative ERCP in the healer to ERCP surgery group(12 cases),direct line of traditional surgery over the same period will be set to direct surgical group(20 cases),14 cases of male,female 18 cases,the male to female ratio: 1:1.29,aged 22 to 68 years old,average 47.06 years old;Comparison in the group: record the successful cases and complications of ERCP in the ERCP operation group,compare the infection indicators and partial liver function changes before and after ERCP;Comparison between groups: comparison of general information,infection index,liver function index,operation time,intraoperative blood loss,postoperative complication rate and postoperative hospitalization days of group A and group B.Results: 12 patients in group A successfully performed ERCP,and 3 of them showed A hyperthermia after ERCP,which improved after treatment.All patients in group A received traditional surgical treatment.Compare A group of patients with ERCP before24 h and 48 h after infection index(WBC,PCT),liver function index(ALT,AST,TBIL,ALP,GGT,),with the exception of ALT,AST,propagated had statistical significance(P <0.05).The general information,infection index and liver function indexes of group A and group B were not statistically significant(P >0.05);There were statistically significant differences(P < 0.05)between group A and group B except for operative time,intraoperative blood loss,postoperative hospitalization days,and postoperative complications.The follow-up time was 6~12 months.In group A,12 patients and 16 patients in group B did not have long-term complications such as in situ recurrence and biliary stricture.Postoperative complications,postoperative complications of group B was significantly higher than that of group A,which caused by acute renal dysfunction and blood coagulation dysfunction of the difference was statistically significant(P < 0.05),and in acute respiratory failure and there was no statistically significant difference of postoperative residual cavity biliary fistula(P > 0.05).Conclusions:For hepatic echinococcosis caused to break into the biliary acute suppurative cholangitis and obstructive jaundice,and applied in preoperative ERCP in treatment plays an important role in the diagnosis and treatment,and preoperative ERCP treatment after undergoing elective traditional surgery in intraoperative blood loss,postoperative hospitalization days and the incidence of postoperative complications is superior to surgical group directly.
