Objective To investigate the application of endoscopic retrograde cholangiopancreatography in the diagnosis and the treatment of acute biliary pancreastitis. Methods A retrospective analysis was made in the data of 153 ABP patients treated by ERCP from January 1,2012 to December 31,2016.According to the time from onset to endoscopic treatment, the course of acute pancreatitis and the actual situation of the patients admitted to our hospital, those patients were divided into group A(treatment <7d group with 42 cases),group B(treatment?7d and treatment <14d group with 45 cases)and group C(treatment?14d group with 66 cases).Analysis was made in the disappearing time of postoperative abdominal pain, the recovery time of serum amylase to normal level, the average days of hospitalization, the hospitalization expenses, and the recovery time of normal diet, the complications, the hospitalization prognosis and the situations of ERCP between the three groups. Results The disappearing time of postoperative abdominal pain, the recovery time of serum amylase to normal level, the average days of hospitalization, and the recovery time of normal diet in group A were shorter thanthose in group B(P < 0.05)and group C(P < 0.05),the hospitalization expenses of group A was less than that of group B and C.The disappearing time of postoperative abdominal pain, the recovery time of serum amylase and diet to normal level in group B were shorter than those in group C(P < 0.05),the hospitalization expenses of group B was less than that of group C.The prognosis of the patients in the three groups after ERCP treatment was improved,no death cases. The differences in the incidence of complications and the prognosis between the three groups were not significant. The stone detection rate of ERCP in ABP patients was 92.15%.The ERCP can find the sediment like stones and negative stones that can not be found by the CT and B-mode ultrasonography. Conclusions The stone detection rate of ERCP was higher than that of CT and B ultrasonography, especially in sediment like stones.ERCP combined with CT or B ultrasound examination could improve the detection rate of stones. The ABP patients who receive ERCP treatment at the early time can achieve faster recovery, lower treatment costs and shorter hospitalization time, also the treamet is effective.
