Objective: To investigate the effect of microsurgical clipping on postoperative complications and prognosis of patients with high grade anterior communicating aneurysms.Methods: Use retrospective analysis of 60 cases of Hunt-Hess III and IV,from January 2010 to May 2017 in our hospital department of neurosurgery after microsurgical clipping surgery for patients clinical anterior communicating aneurysms,according to onset time from the operation time,divided into 30 cases of early(0~3),30 cases of late(more than 8)two sub group,The age of two groups of patients,preoperative Fisher classification,GCS score,and postoperative complications: intracranial hematoma,cerebral vasospasm,delayed ischemic nerve dysfunction,pulmonary infection,hydrocephalus,lower extremity venous thrombosis,cognitive behavior defects and two groups of hospital prognosis were collected,and the data were collected.Statistical analysis.Results: There was no significant difference in age,Fisher classification,GCS,intracranial hematoma and venous thrombosis of the lower extremities between the early and late group(P>0.05).The two groups of hydrocephalus,delayed ischemic nerve dysfunction,pulmonary infection,cognitive impairment and good rate of discharge were statistically significant(P<0.05).Conclusions: Early operation of high grade anterior communicating aneurysms can effectively reduce postoperative complications,and the prognosis is better than that of late operation.
