Study Of Superb Microvascular Imaging Combined With Oral Contrast Ultrasonography In Diagnosing Gastric Lesions

Objective To explore the value of superb microvascular imaging(SMI)combined with oral contrast ultrasonography in diagnosing gastric lesionsMethod SMI combined with oral contrast ultrasonography were performed in 199 patients to observe blood flow of lesion,the benign and malignant of hepatic lesions was diagnosticly analyzed,The ultrasonic test results were compared with the gastroscopy and pathological diagnosis that the lesion was resected,to evaluate accuracy in the differential diagnosis of benign and malignant lesion.Results(1)total of 199 patients were examined by SMI combined with oral contrast ultrasonography,then by gastroscopy and conformed by pathology.82 of 199 were diagnosed as gastritis by SMI combined with oralcontrast ultrasonography then conformed by pathology,missed diagnosis 5 cases,gastroscopy missed diagnosis 2 cases Compared with pathology.25 of 199 were diagnosed as gastric ulcer by SMI combined with oral contrast ultrasonography then conformed by pathology,missed diagnosis 2 cases,gastroscopy missed diagnosis 1 cases Compared with pathology.5 of 199 were diagnosed as gastric polyp by SMI combined with oral contrast ultrasonography then conformed by pathology,missed diagnosis 1 cases,gastroscopy no missed.7 of 199 were diagnosed as gastric stromal tumor by SMI combined with oral contrast ultrasonography then conformed by pathology,gastroscopy missed diagnosis 1cases Compared with pathology.70 of 199 were diagnosed as gastric carcinoma by SMI combined with oral contrast ultrasonography then conformed by pathology,missed diagnosis 4 cases,gastroscopy missed diagnosis 2 cases Compared with pathology.The accuracy of SMI combined with oral contrast ultrasonography was 94.0% in total,97.0% in gastroscopy,respectively.the differences were no statistically significant(P>0.05).(2)SMI flow signals were scored as 0~I in 131 of 135 benign lesions and as II~III in 40 of 64 malignant lesions.The difference between two groups was significant(P<0.05).(3)For small lesions and early gastric cancer(EGC),SMI combined with oral contrast ultrasonography maybe misdiagnosis or miss(4early gastric cancer cases were missed diagnosis in total gastric carcinoma),but it has high accuracy of diagnosis in advanced gastric carcinoma(AGC).Conclusions(1)SMI combined with oral contrast ultrasonography is acheap,non-invasive,non-radioactive method for diagnosing gastric diseases.There was no statistical significance in diagnose accordance rate between SMI combined with oral contrast ultrasonography compare with gastroscopy.SMI combined with oral contrast ultrasonography could be another method to diagnoses gastric diseases besides gastroscopy and barium meal examination.(2)SMI combined with oral contrast ultrasonography has high accuracy of diagnosis AGC,especially in detecting metastasis.But the misdiagnosis rate and omission diagnose rate is high in EGC,gastric polyp,and so on.it should Improve accuracy of diagnose.(3)SMI combined with oral contrast ultrasonography is helpful in differentiating between benign and malignant gastric lesion according flow of blood,and it play a role in assessment of gastric carcinoma pathogenesis progression.
