| ObjectivesThere are three objectives in this study.(1)In order to understand the handgrip strength level and its influence factors in the elderly of Donglan County.(2)To investigate the feasibility of handgrip strength as an indicator of the general health status of the elderly.(3)To provide a reference for further improving the health of the elderly and strengthening the prevention of chronic diseases in the region.MethodsSubjects over 60 years old were recruited in Donglan County(male 881,female 1229).Questionnaire survey was used to collect the general characteristics and daily life information.Health examination was provided for elderly people(including height,weight,handgrip,blood test,chest X-ray,abdominal B ultrasound and other projects).Strength levels were represented with handgrip strength and grip mass index.At the end of the study,the handgrip level and its related factors were analysed using chi-square test,one-way ANOVA and multiple regression analysis methods.Results1.The general characteristics of samples.(1)The effective sample size ofthis study was 2110 people,including 881 males and 1229 females,aged 60~96years.There were no differences in sex and occupation between the groups,both female and peasant oriented.With age increasing,the average height of the elderly decreased,weight loss,bone mineral density decreased,and systolic blood pressure increased.The contents of TC,TG,ALB and HB in each group were statistically different(p<0.05),the changes of TC and TG were not obvious with age,and ALB and HB decreased with age.There was no significant difference in diastolic blood pressure and blood glucose among the different age groups.(2)There were differences in the number of people with different income levels in each group(p<0.05).The annual income of the 60~ group,65~group and 70~ group was mainly 10000~29999 yuan,the 75~ group and 80~group were mainly more than 30000 yuan,and 85~96 years old with annual income of less than 3000 yuan.(3)There were differences in the number of light physical labor,moderate physical labor and heavy physical labor(p<0.05).With the increase of age,the proportion of elderly people engaged in heavy physical labor gradually decreased,and the proportion of light manual workers increased.(4)The weight loss,normal weight,overweight and obesity distribution were statistically significant(p<0.05).With the increase of age,the proportion of underweight people in each group was larger and the overweight was gradually decreased,but most people were normal weight in each group.(5)There were statistically significant differences in normal bone mass,osteopenia and osteoporosis in each group(p<0.05).The proportion of osteoporosis increased gradually with the increase of age.2.The handgrip strength level and characteristics in old population.(1)In this study,there were significant differences in handgrip strength and grip body mass index in the elderly group(p<0.05).With the age growth,the overallhandgrip strength and grip mass index level decreased.(2)The handgrip strength and grip mass index of male were better than female(p<0.05).The level of the two indicators in the male and female elderly people showed a downward trend with the increasing of age.(3)In 65 to 70 years old with annual income of 5000-9999 yuan,their handgrip strength and grip body mass index were higher than other income levels(p<0.05).And other age groups showed no difference(p > 0.05).In addition,the income had no significant effect on handgrip strength and grip mass index.(4)There was no statistically significant difference in handgrip strength and grip body mass index between different age groups the elderly engaged in different levels of physical labor(p>0.05).With the intensity of the physical labor increasing,the handgrip strength and grip mass index did not change significantly.(5)In 60~ years old people,the grip strength of low BMI group were lower than other BMI groups,in addition,there were no significant differences between different BMI groups(p>0.05).But it can be seen that the handgrip strength was increasing with the increase of BMI index.The grip mass index of the same age group decreased with the increase of BMI(p<0.05).(6)The elderly whose ALB、TG、GLU levels in normal had lower grip strength than the abnormal value(p<0.05).The HGS and levels of the elderly with normal SBP、DBP and HB was higher than that of the abnormal values(p<0.05).And other indicators,there were no significant differences in grip strength and grip body mass index with normal value or not(p<0.05).(7)There was a significant difference in handgrip strength between different ages of60-85 years old with different BMD(p<0.05),with the decrease of bone mass,the grip strength decreased.There was a significant difference in grip mass index between different ages of 60-80 years old with different BMD(p<0.05),with the decrease of bone mass,the grip mass index decreased.On the whole,the lower bone mineral density and bone mass,and lower grip strength and grip mass index in the elderly.3.Regression analysis of the influence factors of grip strength.Body weight,farmers,HB,ALB,TC and BMD were positively related to handgrip strength level,while elderly,female,family income,BMI and DBP were negatively related to handgrip strength level.Similarly,farmers,HB,TC and BMD were positively related to gtip mass index,while elderly,female,family income,BMI and DBP were negatively related to grip mass index.Conclusions:1.With the increase of age,the height,weight,ALB,HB and bone density decreased of the elderly population in Donglan County,but systolic blood pressure increased.The proportion of heavy physical workers,underweight and osteoporosis increases with age.2.The grip strength and grip mass index of this study were at a low level.The grip strength level decreased with age.On gender level,the grip strength level of male was significantly better than that of female in the same age.The handgrip strength level was positively related to HB,ALB and BMD,indicating that the better the nutritional status of the elderly,the better the grip strength level.3.Weight,farmers,HB,ALB,TC and bone mineral density were positively related to handgrip strength level,while elderly,female,family income,BMI and DBP were negatively related to handgrip strength level.4.The grip strength level may reflect the nutritional status and Osteoporosis of elderly people to some extent. |
