Objective:To explore the value of contrast-enhanced ultrasound for SHCC diagnosis.Methods:Retrospective analysis the clinical data of 37 patients(41 lesions)with 1?2cm of small hepatocellular carcinoma(SHCC).All patients with liver cirrhosis background,34 lesions diagnosed by surgery or biopsy pathology of HCC,7 lesions diagnosed by clinical diagnosis.Compare the enhancement pattern and the diagnosis result of contrast-enhanced ultrasound and enhancement CT for 1?2cm SHCC,do statistical analysis the difference and consistency of this two kinds of imaging for 1?2cm SHCC diagnosis.Results:Typical arterial enhancement lesion contrast-enhanced ultrasound 35,enhancement CT 31;atypical enhanced lesions Contrast-enhanced ultrasound 6,enhanced CT 10.Arterial enhancement methods comparison,chi-square =1.242,P = 0.3 8.Typical indexes lesion of venous phase and delayed phase contrast-enhanced ultrasound 34,the enhanced CT 30;Atypical indexes of venous phase and delayed phase lesions,contrast-enhanced ultrasound 7,the focus of the enhanced CT 11.Portal phase and delay enhancement method comparison,chi-square = 1.139,P = 0.33.Ultrasonic imaging and enhanced CT in the diagnosis of true positive results of 35 lesions respectively,30 pieces of lesions,false negative,respectively,three,five,you can't judge whether clear for liver cancer lesions respectively for three,six,chi-square=1.885,P=0.07,no statistical differences enhanced method and the diagnosis.typical enhanced Contrast-enhanced ultrasound and enhancement CT arterial lesions are 30 pieces,2 pieces are atypical enhanced lesions,a consistency analysis of arterial enhancement k=0.587,P<0.05;The lesions of portal phase and delayed phase contrast agents present a typical are 25 pieces,1 pieces is not the typical,portal phase and delay to enhance the consistency of the analysis,k = 0.563,P<0.05.Ultrasonic imaging and enhanced CT in the diagnosis of HCC are 29 pieces,the lesions which negatives is 0 pieces,all is not clear diagnosis HCC lesions are 2 pieces,k=0.488,P<0.05.Two types of enhanced imaging examination and diagnosis consistent pattern are all medium.Conclusion:Contrast-enhanced ultrasound and enhancement CT in the diagnosis of 1-2cm of SHCC has a very high accuracy diagnosis level.Contrast-enhanced ultrasound and enhancement CT in the diagnosis of inconsistent results may happen.In clinic,therefore,contrast-enhanced ultrasound can complement to the diagnosis of enhanced CT,for 1?2cm of small liver cancer diagnosis to provide important diagnostic information.If one of two imaging showed typical way of primary liver cancer increase,the lesions should be treated equally like primary liver cancer.
