Objective:To investigate the expression and significance of serum apolipoprotein M(APOM)in type 2 diabetic patients complicated with sepsis.Methods:105 patients with type 2 diabetes mellitus who were treated at Xiangdong Hospital affiliated to Hunan Normal University from January 2017 to December 2017 were selected as the study subjects.According to whether the patients were complicated with infection and the severity of infection, they were divided into three groups.type 2 diabetes mellitus without infection was group B,type 2 diabetes mellitus complicated with general infection was group C and type 2 diabetes mellitus complicated with sepsis was group D.40 healthy people in our hospital were selected as the control group(group A).Blood routine examination,Lipid profile,FBS,PBS,HbA1c,PCT,Hs-CRP,ESR,APOM and other indexes were detected in each group. At the same time, the general information such as sex,age of each group and course of disease were recorded.SPSS 19.0 statistical software was used for data analysis. To explore the expression and significance of apolipoprotein M in type 2 diabetic patients with sepsis. At the same time, the diagnostic value of APOM and PCT and other inflammatory markers in the diagnosis of type 2 diabetic patients with infection and the severity of infection were compared.Results:1.Compared with the healthy control group, the expression of APOM decreased in other groups(all the P values were less than 0.05).Compared with the group of type 2 diabetes without infection, the expression of APOM in type 2 diabetic patients with general infection and sepsis are also decreased(P<0.05).Compared with type 2 diabetic patients with general infection, the expression of APOM in type 2 diabetic patients with sepsis decreased further(P<0.05).The difference was statistically significant.2.Correlation analysis:APOM was positively correlated with HDL and the correlation coefficient was 0.437.APOM is negatively correlated with PCT and the correlation coefficient is-0.590.3.ROC curve analysis:The areas under the ROC curves of APOM and PCT for diagnosing the type 2 diabetic patients with infection were 0.793,0.978 respectively, and the area under the ROC curve of APOM and PCT for identifying the type 2 diabetic patients with sepsis were 0.730,0.783.Conclusion:The serum APOM expression in patients with type 2 diabetes decreased, and when type 2 diabetes patients were infected,it decreased further. The heavier the infection, the lower the expression level of serum APOM,and the higher the PCT.Therefore,it is recommended that the combination of the two can be used to determine the severity of infection in type 2 diabetic patients, which can help early identification of type 2 diabetes with sepsis, and then early intervention to improve the prognosis of the disease.
