Effect Of Allantoin,Metronidazole And Dexamethasone Mixed Lavage On The Recovery Of Gastrointestinal Function After Abdominal Operation

Objective:Abdominal surgery is common in the general surgery,temporary gastrointestinal dysfunction is a process hard to avoid to each patient who has went through the abdominal surgery.How to recover the gastrointestinal function effectively and rapidly,return to normal diet as soon as possible is not only very important and urgent in the healing process of patients but also an important process to the body's rapid recovery and reducing postoperative complications.So this study use the accelerating gastrointestinal function recovery after abdominal surgery as main outcome measurement to evaluat the efficacy of allantoin,metronidazole and dexamethasone mixed lavage of gastrointestinal function recovery after abdominal operation,while evaluating its security.Methods:Double-blind and randomized control research methods are used in this study.Make the experimental drug and contrasted saline the same packaging and dosage forms in the appearance,numbering each experimental drug at random according to the random number table,the researchers give out the experimental drug with the random number according to the selected order of the subjects,so the 48 cases of patients who are conform to the requirements of the group are divided into experimental group of 24 cases and control group of 24 cases,inject allantoin,metronidazole and dexamethasone mixed lavage and saline of the same volume separately before abdomen was closed during the surgery,at the same time put the abdominal cavity drainage tube,closed until two hours.All experimental patients are forbidden to use PCA pumps and give 5mg morphine intramuscular injection each time if the pain is unbearable,record the morphine consumption with 72 hours after surgery at the same time.Record the first time of fart,defecation and solid food endurance time(solid food including normal diet,soft food and semi-liquid food is intaken of 50g and nausea or vomiting does not occur within 4 hours),also record the postoperative hospital stay(the end of surgery to the cure standard time the researchers believe instead of to the discharge notice time).Check and record the postoperative inflammation indicator and security metrics.Apply soft ware SPSS 21.0 to perform statistical analysis and evaluate the efficacy and safety of allantoin,metronidazole and dexamethasone mixed lavage when the experiment is finished.Results:1 There were no significant statistical difference in general information:operative time,the amount of bleeding and incision length,etc of the two groups of patients(P>0.05).2 There were statistical difference in the time of first fart,first defecation,solid food endurance time and postoperative hospital stay(P<0.05),the manifestation is that the experimental group is earlier in the time of first fart,first defecation,solid food endurance time and postoperative hospital stay than the control group.3 The CRP of the experimental postoperative group is lower than the control group on the first and third day(P<0.05),but there was no significant difference of the CRP on the the first day before surgery,and the postoperative seventh day(P>0.05).4 Peritoneal exudate within 24h of the experimental postoperative group experimental group is lower than the the control group(P<0.05)and even if there is no statistical significance within 48h(P>0.05),the peritoneal exudate within 48h of the experimental postoperative 'group is obviously lower than the control group.5 No statistical differences were found in the amount of the peritoneal exudate leukocyte of the two groups,but the average of the experimental group was significantly lower than that of the control group,therefore,the experimental group trend to downward.6 There was no significantly statistical difference in the indicators of blood,liver function,kidney function,blood clotting function and other indicators of the two groups of patients(P>0.05).Among all the patients,there were three patients getting the lung infection,one in the experimental group,the rest of the patients in the control group,who were cured after the active treatment.No died cases during surgery and hospital stays and no cases appeared irreversible serious complications.Conclusion:Allantoin,metronidazole and dexamethasone mixed lavage can promote gastrointestinal function recovery after abdominal operation,while it is safety and effective,in addition,it is not serious adverse reactions.
