BackgroundMyocardial infarction is one of the major cardiovascular diseases that cause sudden death in today's society.With the urgent need of high quality living standard in our country,a series of health problems such as sudden death caused by myocardial infarction need to be solved urgently.Because myocardial infarction is a complex and variable cardiovascular disease,the risk factors for myocardial infarction include not only genetic factors,but also some environmental factors,such as age,smoking,hypertension,diabetes and hyperlipidemia.These independent or common risk factors and coronary artery atherosclerosis is closely related to the formation of tenderness,and ultimately the development of myocardial infarction.Although the diagnosis and treatment of myocardial infarction have been continuously improved,these risk factors do not explain the epidemic and disease characteristics of all myocardial infarction.Data show that virus and bacterial infections can also cause atherosclerosis in susceptible populations.The periodontal disease is a chronic infectious disease,mainly for inflammatory disease pathogens in bacteria and their products infected gums or periodontal tissue into the blood circulation system,possibly through its release of inflammatory factor vascular endothelial cell injury,leading to the formation of coronary atherosclerosis.Therefore,this study focuses on the correlation between periodontitis and myocardial infarction.ObjectiveMeta analysis was used to evaluate the correlation between periodontitis and myocardial infarction.MethodsObservational studies(cohort study,case-control study,cross-sectional study)published on a computer-based search of published PubMed,ChinaNet and Wanfang database to investigate the association between periodontal disease and myocardial infarction were performed at a search time of Establishment of a database as of January 31,2017(updated on February 7,2017)Two independent researchers conducted a literature review,data extraction,and risk assessment of bias,and conducted a case-control study and a quality assessment tool for cohort studies Noc(Newcastle-Ottawa Scale)scale and cross-sectional study evaluation tool Combie scale for quality evaluation,using the latest version of statistical software R R3.3.3 for meta-analysis.ResultsThe final meta-analysis included 32 eligible national and international articles that covered 10 different countries on 3 continents and all of the literature topics related to periodontal disease and myocardial infarction.Due to the various documents from different countries,so there are some differences in the literature led to the Meta analysis of some of its own limitations.Meta-analysis of the final results showed that: due to the heterogeneity of the inclusion of the literature,so the use of random effects model for all literature OR of the effect of the combined data showed that periodontal disease increased risk of myocardial infarction 2.09 times(OR = 2.09,95% CI = 1.75-2.48,P <0.01).ConclusionBased on the meta-analysis,the available evidence indicates that periodontal disease is associated with the occurrence and development of myocardial infarction and that the results of statistical analysis of data demonstrate that periodontal disease increases the risk of myocardial infarction.Conclusions are summarized as follows:(1)Asians exposed to periodontal disease increased the risk of myocardial infarction by 2.52 times(OR = 2.52,95% CI = 1.17-5.46,P = 0.02).(2)Caucasians exposed to periodontal disease increased the risk of myocardial infarction by 2.06 times(OR = 2.06,95% CI = 1.69-2.51,P <0.01).
