Objective To analyze the kinematic and the surface electromyography parameters of lower limb and the correlation with gait function in stroke patients with knee hyperextension,and to explore the main factors that influence walking ability of patients with knee hyperextension,and find the key points of the intervention in rehabilition therapy.Methods To compare the kinematic and surface electromyography paremeters between26 cases of hemiplegia patients with knee hyperexttension and 20 cases of healthy elderly people by three-dimensional gait analysis system.And anglysis the correlation between gait function and kinematic?surface electromyography parameters.Results Compared to the control group,there were significant differences(P<0.05)in spatiotemporal(Steplenth,Steps Per Minute,Stance Percent,Speed)and kinematic parameters(The joint range of motion of hip?knee and ankle,especially the maximal extension of hip?the flexion and extension of knee and the dorsiflexion of ankle)in the stroke patients with knee hyperextension gait.Compared with the unaffected limbs,there were significant differences(P<0.05)in kinematic parameters(Stance Percent,The joint range of motion of hip and knee,especially the maximal flexion of hip,the flexion and extension of knee and the dorsiflexion of ankle)in the affected limbs of stroke patients with knee hyperextension gait.There were significant correlation(P<0.05)between the gait function and kinematic parameters(Steps Per Minute,Stance Percent,The ROM of hip?knee and ankle,especially the flexion of knee and the plantarflexion of ankle)in patients with knee hyperextension gait.Compared to the control group,there were significant differences(P<0.05)in the RMS of gluteus maximus,vastus medialis and gastrocnemius.Compared with the unaffected limbs,there were significant differences(P<0.05)in vastus medialis,tibialis anterior,gastrocnemius and soleus.There were significant correlation(P<0.05)between the gait function and the RMS of gastrocnemius.Conclusion 1?The lower joint range of motion were restricted in hemiplegic patients with knee hyperextension,especially the maximal extension of hip,the flexion of knee and the plantarflexion of ankle.2?Except the gluteus maximus,the muscles' strength of the bilateral lower limbs were declined in hemiplegic patients with knee hyperextension,especially the vastus medialis,gastrocnemius and soleus.3?The restrict-ion of knee's flexionand the ankle's plantarflexion,the function of gastrocnemius were important factors of influencing walking ability in stroke patients.4?The weakness of the vastus medialis and the spastic of the gastrocnemius may be the main mechanism of the knee-hyperexttension.5?The three-dimensoonal gait analysis system can be a valua-able tool for testing the gait of post-stroke hemiplegic patients with knee hyperexttension and serve as a reference for planning rehabilitation interventions and evaluation of therapeutic effect for such patients.
