Objective By measuring the serum leves of ecreted frizzled-related protein 5(sfrp5)and wingless MMTV integration family members 5?(Wnt5?)in metabolically healthy obese(MHO)individuals,to understand their variation and clinical significance in the MHO population.Methods Epidemiological survey data of 1426 permanent residents(living in at least 5 years)in a certain community in Hefei City in 2016 were collected,including questionnaires.Height,body weight,waist circumference,blood pressure,fasting blood glucose(FBG),serum cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein-cholesterol Glycosylated hemoglobin(Hb A1c),and 2 hours postprandial blood glucose(2 h PBG)was measured.Serum levels of sfrp5 and Wnt5? were determined by enzyme-linked immunosorbent assay(ELISA).According to the inclusion criteria of MHO and obesity with metabolic syndrome(MS),150 subjects were divided into normal group(n = 50),MHO group(n = 50),obesity with metabolic syndrome(MS)group(n = 50).By analyzing and comparing the general data of three groups of patients,topreliminary explore variation and clinical significance of serum sfrp5 and Wnt5? in the MHO populationin.Results(1)No significant difference of the leves of age,gender and low density lipoprotein cholesterol(LDL-C)could be drawn among the three groups(P >0.05);Compared with the normal group,the BMI and waist circumference in MHO group were significantly increased(P<0.01),and there was no significant difference in systolic blood pressure,diastolic blood pressure,FBG,2h PBG,TG,TC,HDL-C and Hb A1c(P >0.05).Systolic blood pressure,diastolic blood pressure,TG and TC in MHO group were higher than those in normal group,while HDL-C showed a decreasing trend;Compared with the normal group,BMI,waist circumference,systolic pressure,diastolic pressure,FBG,TG,TC of obesity with MS group were significantly increased(P <0.01),HDL-C was significantly decreased(P<0.01),and 2h PBG,Hb A1 c were increased(P<0.05);Compared with obesity with MS group,systolic blood pressure,diastolic blood pressure,FBG,TG and TC in MHO group were significantly decreased(P<0.01),HDL-C significantly increased(P<0.01),and 2h PBG,Hb A1 c were increased(P<0.05);while BMI,waist circumference were no significant difference(P >0.05).(2)The levels of sfrp5 in MHO group and obesity with MS group were significantly lower than those in normal group(P <0.01).The level of sfrp5 in MHO group was significantly higher than that in obesity with MS group(P <0.01).(3)Compared with the normal group,the level of serum Wnt5? in MHO group was significantly increased,the level of serum Wnt5? in obesity with MS group was significantly increased,and the serum Wnt5? level was not statistically different between the two groups(P >0.05).(4)Compared with the normal group,the ratio of Wnt5?/ sfrp5 in MHO group was increased,and the Wnt5?/ sfrp5 ratio in obesity with MS group was significantly increased.The ratio of Wnt5?/ sfrp5 in MHO group was significantly lower than that in obesity with MS group(P <0.01).(5)Pearson correlation analysis showed that serum sfrp5 was positively correlated with HDL-C(r = 0.283,P <0.01)and negatively correlated with BMI,waist circumference,systolic blood pressure,diastolic blood pressure,FBG,2h PBG,TG.There was a positive correlation between Wnt5? and BMI and waist circumference(r = 0.285,0.281,P<0.01);Wnt5?/ sfrp5 and BMI,Waist circumference,systolic blood pressure,diastolic blood pressure,TG and TC (r = 0.499,0.487,0.346,0.285,0.339,0.302,P<0.01),but negatively correlated with HDL-C(r =-0.345,P<0.01).(6)Serum sfrp5 was negatively correlated with serum Wnt5?(r =-0.741,P<0.01).(7)Multivariate linear stepwise regression analysis showed that BMI,2h PBG and TG were the main influencing factors of serum sfrp5.BMI was the main influencing factor of serum Wnt5?.The main influencing factors of Wnt5? / sfrp5 were BMI,TC,HDL-C and Hb A1 c.Conclusion(1)The levels of serum sfrp5,Wnt5? and Wnt5?/sfrp5 in metabolically normal obese individuals were all in the normal population and between those with obesity and metabolic syndrome,suggesting that the metabolically normal obese population was a transitional state between them.(2)There is a correlation between serum sfrp5 and serum Wnt5?,and there is a certain correlation between serum sfrp5,Wnt5? and Wnt5?/sfrp5 and BMI,blood glucose,lipids and other indicators,suggesting that both may be one of the specific mechanisms affecting glucose and lipid metabolism.(3)The maintenance of metabolic normality in metabolically normal obese people may be related to the protective effect of serum sfrp5 and the weakened mechanism of serum Wnt5?,especially serum sfrp5.Therefore,the determination of Wnt5?/sfrp5 ratio may be of great value in determining the prognosis and prognosis of MHO patients.
