The Exploration Of The Pathway Of Pharmaceutical Care For Patients With Atrial Fibrillation

Objectives Based on the implementation of clinical pathways,to study drug development pathways of atrial fibrillation patients to develop and implement methods To ensure the safety of the patient during the medication while ensuring rational drug use.Methods The author reviewed the data and retrieved 839 cases of department of Internal Medicine with atrial fibrillation in the year of 2016.The patients with atrial fibrillation had reached 23.6%.After determining the type of disease,according to the previous treatment experience,the first thing to do is to formulate the path of pharmaceutical care for patients with atrial fibrillation,divide the patients with consistent condition into two groups.We take the patients in the period form October to December in 2017 who meet the condition of atrial fibrillation as control group.And We take the patients in the period form January to February in 2018 who meet the condition of atrial fibrillation as intervention group,.In the intervention group,the pharmacist will participate in the patient's treatment process and conduct medical supervision,including drug education,medication guidance,drug administration program and analysis of adjusted drug use,adverse reaction monitoring,drug discharge from the hospital,and medication guidance for discharge from the hospital.The pharmacist will sum up its pharmaceutical care path.Analyze patients with atrial fibrillation with statistical software such as SPSS 19.0,evaluate the effect of pharmacy services on the average length of hospital stay,hospitalization costs,adverse drug reaction,dosage regimen and analysis of adjusted medications,medications discharged from the hospital,and medication instructions for discharge from hospitals.Results We have developed a list of patients with atrial fibrillation of the drug monitoring path,and have summarized the points of pharmacy care for patients with atrial fibrillation during hospitalization.During the period from October 2017 to February 2018,a total of 50 patients with atrial fibrillation were treated with pharmacological intervention.The results showed that the average length of hospital stay,hospitalization costs,incidence of adverse reactions in patients with atrial fibrillation decreased significantly,and drug compliance significantly increased.Conclusion Pharmaceutical Care can promote the rational use of drugs in clinics,and can ensure the safety of patients' medications to improve the therapeutic benefit of patients.Pharmacy care makes the patient's treatment process more standardized and also greatly improves patient satisfaction.At the same time provide pharmacists with better service ideas.
