Objective:To explore the relationship among the preoperative inflammation index and the postoperative Ki-67 index and the pathological grade of clear cell renal cell carcinoma.Method:1.Collect 60 cases of clear cell renal cell carcinoma(CCRCC)pathologically diagnosed in our hospital.According to the 2016 WHO International Urological Pathology Society classification,the pathological grade results are divided into G1 + G2 and G3 + G4 levels group.2.Preoperative inflammatory markers were collected from patients with CCRCC and 40 healthy people during the same period.Inflammatory markers included neutrophil-lymphocyte-ratio(NLR),lymphocyte-monocyte-ratio(LMR),platelet-lymphocyte-ratio(PLR)and red blood cell distribution width(RDW).3.Using the ROC curve to calculate the optimal sensitive cutoff values of preoperative inflammatory markers NLR,LMR,PLR and RDW in patients with CCRCC.The relationship between the sensitivity thresholds of four inflammatory markers and the pathological stage of CCRCC was analyzed.4.Immunohistochemistry staining was used to detect the expression of Ki-67 in 60 cases of CCRCC.The expression intensity was divided into weak positive,positive and strong positive.5.Analysis of Ki-67 expression and the relationship between the two groups of pathological grade CCRCC;analysis of Ki-67 expression and CCRCC patients preoperative inflammatory index of the best sensitive cut off the relationship between the values.Result:1.Using the ROC curve to calculate the best sensitive cut-off value of NLR was 2.71,the sensitivity was 51.3%,and the specificity was 62.0%;The best sensitive cut-off value of LMR was 3.18,the sensitivity was 60.0%,and the specificity was 85.0%;The most sensitive cut-off value of PLR was 125.61,the sensitivity was 70.0%,and the specificity was 90.0%;The average RDW was 13.07,the sensitivity was 98.0%,and the specificity was 85.0%.2.Using Spearman correlation test method,the analysis results showed that there was a positive correlation between NLR,PLR and the pathological stage of CCRCC(correlation coefficients were 0.372,P<0.05),while there was no significant difference in pathological staging between LMR,RDW and CCRCC(P>0.05).3.The expression rate of Ki-67 in CCRCC was 100%,weak positive was 58%,positive was 18%,strongly positive was 10%and Ki-67 expression intensity was positively correlated with the pathological stage of CCRCC(correlation coefficient was 0.625,P<0.05).4.There was no significant difference in the expression of NLR,PLR,LMR,RDW and Ki-67(P>0.05).Conclusion:1.The preoperative inflammatory markers NLR and PLR were associated with the pathological stage of CCRCC.2.The expression intensity of Ki-67 was positively correlated with the pathological stage of CCRCC.3.Ki-67,NLR and PLR were independent factors of the pathological stage of CCRCC. |
