| Objective:Comparative analysis of color doppler flow imaging and CT guided percutaneous biopsy in the diagnosis of pulmonary peripheral lesions diagnostic rate,puncture time consuming,and incidence of complications and discuss the related factors,to understand the different in the clinical application value.Method:The retrospective analysis was conducted on 152 patients hospitalized in Anqing Hospital affiliaited to Anhui Medical University from January in 2013 to January in 2017.The patients were divided into two groups according to the different methods of percutaneous biopsy.One group was real-time guided by CDFI(119 cases),and the other was ct guided group(33 cases).Analyze the differences and reasons between the two groups in diagnosis success rate,puncturing time and incidence of complications comparatively.To understand the different meanings of the two in the diagnosis of pulmonary peripheral lesions in ord to guide that clinical work.Result:1、There were 152 patients in the two groups,including 119 patients with color doppler flow imaging,and the success rate was 100%,and the diagnostic success rate was 89.1%.Among them,71 cases were malignant,35 cases were benign,3 cases were suspected,and 9 cases of pathological diagnosis were not consistent with the final clinical diagnosis,and 1 case was lost.CT guided group of 33 patients,based on the success rate of 100%,there are 29 cases made pathological diagnosis and surgical pathologic results conform or consistent with the clinical final diagnosis,diagnosis rate 87.9%,23 cases of malignant,benign in 5 cases,and 4 cases of pathological diagnosis and clinical diagnosis in the end.2、Two groups of diagnostic success rates were compared:the diagnostic success rate of the color Doppler flow imaging guided group(89.1 %)was higher than that of CT guided group(87.9 %),but there was no significant difference between them(p < 0.05).3、The two groups of puncture time were compared:the puncturing time of the color Doppler flow imaging guided group(13.9±6.8 minutes)was less than that in the CT guided group(32.2±11.6 minutes).The difference between the two groups was statistically significant(p<0.05).4、The incidence of complications in the two groups was compared:there were no serious complications in the two groups,such as pneumothorax,hemoptysis and pleural effusion,and the incidence of complications in CDFI guided group(5.8 %)was lower than that of CT guided group(39.4 %),which was statistically significant(p < 0.05).Conclusion:1.The Color doppler flow imaging(CDFI)guided was more successful than CT guided percutaneous biopsy,and the diagnostic success rate was 89.1% and 87.9% respectively.Both of them are effective methods for the diagnosis of peripheral pulmonary disease,and the Color doppler flow imaging guidance group has a higher success rate.2.The color doppler flow imaging(CDFI)shorter than that of CT-guided percutaneous lung biopsy in the average time-consuming,the average time of the color doppler flow imaging guided group about was(13.9 ± 6.8)minutes,and the CT guided group was(32.2±11.6)minutes,and the color doppler flow imaging guided group is faster.3.The complications of color doppler flow imaging guided group were lower than the ct guided group.The total complication rate of color doppler flow imaging group was 5.8%,and the ct guidance group was 39.4%.The incidence of pneumothorax color doppler flow imaging guided group was about 4.2%,and ct guided group was 33.3%.The incidence of bleeding color doppler flow imaging guided group was about 1.6%,and ct guided group was about 6.1%.The color doppler flow imaging leads to lower incidence of complications and is safer.4.In addition to the advantages of low time and low complication rate,color doppler ultrasound has no radiation and can be easily checked by bedside.Therefore,the color doppler flow imaging guided percutaneous lung puncture is the first choice to diagnose the lesions around the lung. |
