The New Clinical Treatment For Repeated Failure Of Embryo Implantation And Recurrent Spontaneous Abortion

Objective: 1.To investigate the value of preimplantation genetic screening(PGS)in elderly women and recurrent spontaneous abortion(RSA)patients.2.Through the immunotherapy with immunoglobulin for infertility patients with recurrent IVF-ET failure,to explore the effect of the treatment of infertility applied immunoglobulin.Methods: 1.91 elderly and RSA patients who were treated with PGS in navy general hospital of PLA were selected as the study objects.Single cell whole genome amplification combined with second-generation sequencing technique was used to screen 24-chromosomes of the embryobiopsy,and the embryos with normal chromosomes were selected for transfer to observe clinical outcome.2.77 infertility patients with recurrent IVF-ET failure carried 84 embryo transfer cycles were collected.Aged 36.55±4.61.under 35 years group of 24 people,36-39 years group of 30 people,over 40 years group of 23 people.All of the patients intravenous immunoglobulin treatment,then after conventional embryo transplantation,to determine whether pregnant.Results: 1.The rate of abnormal embryos in the 526 embryos of these 91 patients with PGS was 64.63%(340/526).The rate of abnormal embryos in RSA patients is 65.96%(217/329)and clinical pregnancy rate was 56.00%(28/50),The rate of abnormal embryos in elderly patients is 72.84%(177/243)and clinical pregnancy rate was 57.14%(16/28).2.Clinic pregnancy rate of 77 patients of applied immunoglobulin was 50.00%(42/84)、inplantation rate was 70.24%(59/84).Pregnancy rate of under 35 years group was 55.56%(15/27)、inplantation rate was74.07%(20/27).Pregnancy rate of 36-39 years group was 64.52%(20/31)、inplantation rate was 77.42%(24/31).Pregnancy rate of over 40 years group was 26.92%(7/26)、inplantation rate was 57.69%(15/26).Conclusion: 1.High rate of aneuploidy in embryo had been found in olderly patients and/or RSA patients.These patients’ clinical pregnancoutcome can be improved by preimplantation genetic screening.2.Applied immunoglobulin treatment of infertility patients with recurrent IVF-ET failure has certain curative effect,and prompt part of infertility patients with recurrent embryo transfer failure may have immune factors.
