| Objective To investigate the effects of simutaneous multi level surgery for patients with moderate or severe obstructive sleep apnea hypopnea syndrome,and to evaluate the change in the sleep architecture after the surgery.Methods 36 cases(aged from 27 to 61 years,mean 40.51±8.0 years)who diagnosed with moderate or severe OSAHS,underwent simultaneous multi level operation treatment,were included from E.N.T&H.N.department of Tongling People’s Hospital,dating from Aug 2013 to May 2017.The patients were followed up for six months,who refilled Epworth sleepiness scale(ESS)and rechecked polysomnography(PSG).We evaluate the effect of surgery through the PSG,ESS and the change in the sleep architecture before and after the treatment.Results From postoperative 6 months,the average AHI and ESS score were significantly decreased.The average LSa O2 value was obviously increased.Percentage of rapid eye movement(REM)during TIB were increased.Percentage of N1 and N2 during TIB was decreased while percentage of N3 during TIB was increased.The postoperative AHI 、LSa O2 and ESS were obviously improved before the preoperation.In the 36 followed-up patients,three patients were completely cured,the cured rate was 8.33%;22 patients were markedly effective,the significant efficiency rate was 61.11%;eight patients were effective,the effective rate was 22.22%;and three patients were completely noneffective,the noneffective rate was 8.33%;the total effective number was 33 patients,the total effective rate was 91.67%.Conclusions The coblation assisted simultaneous multi level operation treatment is a safe and feasible therapy for adults with moderate or severe OSAHS,who refused or couldn’t tolerate n CPAP.But we need to select the surgical modality individually for different patients.At same time,it reduces the pain and fear of patients,shorts the hospitalization time,and saves the economic resources. |
