[Background and purpose]Acute coronary syndrome is a serious heart disease caused by coronary atherosclerotic lesion.It is the main cause of cardiovascular disease.It can be classified into unstable angina pectoris and acute myocardial infarction according to severity.Currently,clinically recognized indicators of high expression in serum of ACS patients are Brain natriuretic peptide and myocardial necrosis markers,such as cardiac troponin I?Therefore,this study selected in our hospital ACS patients,stable angina and healthy people as the research object,the detection of population related serum marker expression level of B type natriuretic peptide and cardiac troponin I and miRNA,to investigate the expression level in the serum of patients with ACS in the above indices and their correlation.Therefore,this study selected in our hospital ACS patients,stable angina pectoris(stable angina,pectoris,SAP)and healthy people as the research object,the detection of population related serum marker expression level of B type natriuretic peptide and cardiac troponin I and miRNA,to investigate the expression level in the serum of patients with ACS in the index and the relationship between.It is helpful for early detection of ACS patients and timely treatment for them,in order to reduce the risk of heart failure,malignant arrhythmia and other high risk complications and even death,so as to improve the short-term and long-term prognosis of patients.[Method]A total of 80 ACS patients(including 40 patients with AMI and 40 patients with UAP)who were admitted to the Department of Cardiology,Sichuan Science City Hospital from October 2015 to March 2017 were consecutively recruited.40 patients with SAP and 40 healthy subjects were selected as study subjects.All selected participants received 5 m L of peripheral venous blood on a fasting basis after hospital admission.Serum BNP and cTNI levels were measured by solid-phase immunochromatographic assay.Real-time PCR was used to detect miRNAs(miR-208 b,miR-378 and miR-499)levels.[Results]The serum of group ACS were BNP,cTNI,miR-378,miR-499 and miR-208 b levels in [AMI group were: 332.22(216.11-428.33),26.23(19.82-38.35),4.86(3.13-6.45),4.09(2.89-5.14),3.14(2.59-3.71)];UAP group were: 207.88(113.94-298.82),3.45(1.73-5.18),2.84(1.99-3.42),1.81(1.32-2.29),2.09(1.44-2.48)] was significantly higher than that of SAP group [58.49(29.25-87.85),1.14(0.57-1.71),1.84(1.64-2.09),1.18(0.68-1.51),1.21(0.72-1.48)] and control group [18.09(9.05-27.15),1.19(0.60-1.79,1.62(0.92-1.97),0.97(0.46-1.58),1.14 the corresponding index(0.59-1.36)] in serum,the difference was statistically significant(P < 0.05);each index between every 22 compared with a certain correlation(P < 0.05);two yuan Logistic retrospective analysis results showed that: BNP[OR(odd ratios)4.73(Confidenc,95% E Intervals CI 1.83-12.24])and cTNI(OR:3.16,95%CI:1.43-6.26),miR-378(OR:5.96,95%CI:1.46-24.32),miR-499(OR:1.54,95%CI:1.08-2.20)and miR-208b(OR:4.37,95%CI:2.07-9.22)has a good predictive value for the occurrence of ACS(P < 0.05).[Conclusion]Compared with healthy people and SAP patients,the levels of BNP,cTNI,miR-378,miR-499 and miR-208 b in serum of ACS patients(including AMI and UAP)increased,and the most significant increase was in AMI patients.The above index detection can be used as an important serological marker to predict the occurrence of ACS,and the combined detection can improve the diagnostic value.
