Objective:Coronary heart disease is a heart disease caused by coronary atherosclerosis,which affects blood supply and oxygen supply.Hyperlipidemia is a risk factor for coronary heart disease.The pathogenesis is activation of inflammatory response pathways,and adaptive immunity,characterized by lymphocyte involvement plays an important role in its development.Periodontal disease is an infectious disease of periodontal tissue caused by plaque microorganisms.Periodontal pathogen and its metabolite endotoxin can activate immune reaction cells such as mononuclear/macrophage,which increase the permeability of blood vessels,secrete and release specific inflammatory mediators.It can not only cause damage to the structure and function of local periodontal tissues,but also can invade the endometrial cells through blood circulation,which can lead to atherosclerosis.This leads to the adhesion and accumulation of platelets,which promotes the deposition of oxidized low-density lipoprotein,thereby accelerating the occurrence and development of coronary heart disease.Therefore,this study focused on the statistical analysis of inflammatory factors and lipid levels of periodontitis in patients with coronary heart disease.Choose the the common inflammatory factors that promote the occurrence and development of periodontal disease and coronary heart disease include c-reactive protein?CRP?,interleukin?IL-6?,tumor necrosis factor-??TNF-??.The clinical data and biological samples of the research population were established by collecting coronary heart disease patients hospitalized in the cardiology department,who were performed the periodontal examination.Blood lipid index of high-density lipoprotein-cholesterol?HDL-C?,low-density lipoprotein-cholesterol?LDL-C?,total cholesterol?TC?,triglyceride?TG?and inflammatory markers of coronary heart disease such as CRP,TNF-?,IL-6 associated with periodontal disease were tested.By multivariate statistical analysis of canonical correlation analysis,principal component analysis,the whole subset regression and small sample OLS regression,the statistical data of correlation between inflammation factors and blood lipid level in coronary heart disease and periodontal disease were obtained.Epidemiological analysis of the clinical and biological relevance of inflammatory factors,periodontitis and coronary heart disease become possible.It is of great significance to effectively preserve periodontal tissue health and prevent periodontitis related coronary heart disease.Methods:1.Research objectSelecting from June 2016 to November 2016,65 patients with cardiology at the Second Affiliated Hospital of Dalian Medical University,including 32 males and 33females,aged 41 to 84 years,excluded from heart,liver,kidney insufficiency,diabetes,infectious diseases,periodontal treatment for nearly 1 month and other cases that do not meet the inclusion criteria.2.Statistical variablesThe contents of the questionnaire mainly include:age,gender,economic conditions,education1level,1native1place,1past1hypertension,1family1history1of cardiovascular and cerebrovascular diseases,and the nephrotoxic drugs in use,history of hyperlipidemia and diabetes,high fat eating habits,oral health status,alcohol and tobacco history,knowledge,attitude,behavior,and primary prevention knowledge of coronary heart disease and periodontal disease,current oral hygiene habits,occupation,periodontal treatment and blood pressure and body mass index?BMI?were recorded.3.Periodontal examinationAll of the patients were examined for a complete periodontal examination in all teeth using Williams periodontal probe?third molars were excluded?.Each tooth was examined for 6 sites?distal buccal,distal lingual,mesial buccal,mesial buccal,straight lingual,straight buccal?and all of the examinations were done by the same dentist,recording the probing depth,gingival recession,clinical attachment level,bleeding on probing and observing the tooth mobility,tooth loss,furcation involvement.4.Detection of blood lipid levels and inflammatory factorsAn automatic blood biochemical analyzer was used to measure blood lipid levels,including total cholesterol?TC?,low-density lipoprotein-cholesterol?LDL-C?,triglyceride?TG?,and high-density lipoprotein-cholesterol?HDL-C?.The ELISA kit was used to detect inflammatory factors related to coronary heart disease and periodontal disease,including serum c-reactive protein?CRP?,tumor necrosis factor-??TNF-??,and interleukin-6?IL-6?.5.Statistical methodIn this study,databases were established using Epidate 3.0.Continuous variables of approximately normal distribution were described by?x?s?,which including age,body mass index?BMI?,high-density lipoprotein-cholesterol?HDL-C?and serum c-reactive protein?CRP?.M(Q25Q75)is used for the inflammatory factors which are obviously skewed distribution such as total cholesterol?TC?,triglyceride?TG?,low-density lipoprotein-cholesterol?LDL-C?,tumor necrosis factor-??TNF-??,and interleukin-6?IL-6?.Multivariate statistical analysis such as canonical correlation analysis,principal component analysis,full subset regression and small sample OLS regression were used to investigate the association between inflammatory factors and blood lipid levels.The ANOVA or chis square test were applied to investigate the association of periodontitis with serum lipoprotein and inflammatory factors.All of those analysis were performed on IBM Statistic SPSS 22.0 statistical software and P<0.05 was regarded as significant level in our study.Results:1.With the increase of the severity of periodontitis,the level of HDL-C decreased significantly?p<0.01?,and the serum CRP,IL-6,TNF-?level showed an increasing trend?p<0.05?.2.In the indicators of blood lipid level factors,the level of the index of HDL-C was dominant;In the indicators of serum inflammatory factors,the level of the index of IL-6 was dominant;The level of HDL-C and IL-6 was negatively correlated relationship.3.The effects of TC on the three explanatory variables CRP,IL-6 and TNF-?were positive,and when the other explanatory variables remained unchanged,the CRP change was 2.148%for every 1%change in total cholesterol,causing IL-6 changes of3.374%caused a 3.638%change in TNF-?.Conclusion:1.Under this experimental condition,the serum CRP,IL-6,TNF-?level werepositively correlated with the severity of periodontitis in patients with coronaryheart disease.The HDL-C level was negatively correlated with the severity ofperiodontitis in patients with coronary heart disease.2.Under this experimental condition,the effects of TC in patients with coronary heartdisease on the three explanatory variables CRP,IL-6 and TNF-?were positive.Thecontent of HDL-C was dominant in the indexes of blood lipid level and the contentof IL-6 is dominant in the indexes of the inflammatory factors.The level ofHDL-C and IL-6 was negatively correlated relationship.3.Inflammatory factors and blood lipid levels related to coronary heart disease andperiodontitis have a certain correlation.The examination of periodontitisinflammatory factors may be a new method for the screening of coronary heartdisease.
