Objective: In the artificial hip arthroplasty,the traditional biological fixed femoral prosthesis was widely used,and the clinical effect was good in the near future and foward.In this study,the early clinical efficacy of Tri-lock BPS(Johnson &Johnson,Depuy)for hip arthroplasty was discussed by clinical and radiological evaluation.Methods: This study selected retrospectived patients with total hip arthroplasty for the first application of Corail or Tri-lock BPS stem from January 2015 to December 2016 in our hospital.Collected the infomation of patients' gender,age,body mass index(bmi),preoperative diagnosis,preoperative and postoperative Harris hip score,intraoperative situation.Statistical analysis was performed on the data.Preoperative diseases included necrosis of femoral head,hip dysplasia and osteoarthritis.Follow-up x rays were taken on the day after surgery,2 weeks after surgery,1 month after surgery,3 months after surgery,6 months after surgery and every year after surgery.Patients using Corail stem were 36 cases(36 hips),20 cases were male,16 cases were female.The average age was(64.27 + 3.07)years old,and the average body mass index(bmi)was(24.36±1.2)kg / ?.According to Dorr classification,there were 16 Dorr A types and 20 Dorr B types in Corail group.27 cases were diagnosed as femoral head necrosis,5 cases were diagnosed as hip dysplasia,and 4 cases were diagnosed as osteoarthritis.There were 29 patients(29hips)using Tri-lock BPS stem,14 cases were male,15 cases were female.The average age was(62.4 + 2.15)years old.The average body mass index(bmi)was(24.03 + 1.43)kg / ?.21 cases were diagnosed as femoral head necrosis,4 cases were diagnosed as hip dysplasia,and 4 cases were diagnosed as osteoarthritis.Tri-lock BPS group includeed 16 cases belonged to Dorr A and 13 cases belonged to Dorr B.Compare the differences between the two groups in the surgical procedure,hip function and postoperative imaging.SPSS20.0 statistical software was used to analyze the data.Results: The average follow-up time of the Corail group was 12 months,and the Harris score of the hip joint was increased to(90.53 + 5.26)points at the final follow-up from the preoperative(38.23 + 4.91)points.The rate of excellet and good results was 86%.The average follow-up time of the Tri-lock BPS group was 11 months,and the Harris score of the hip joint was increased to(91.53 + 6.27)points at the final follow-up from the preoperative(42.50 + 2.73)points.The rate of excellet and good results was 90%.Operation time of the Corail group was(65.41 + 6.24)min,intraoperative blood loss was(189.20 + 59.14)ml,postoperative drainage was(217.63+ 23.27)ml.Operation time of the Tri-lock BPS group was(62.83 + 4.81)min,intraoperative blood loss was(151.20 + 51.24)ml,postoperative drainage was(203.2 +18.39)ml.There was no significant difference in hip function improvement,operation time,intraoperative blood loss and postoperative drainage.All patients showed no joint infection,joint dislocation,prosthesis subsidence,periprosthetic fracture,vascular or nerve injury and other complications.No deaths were reported during follow-up.During the follow-up period of the Corail group,there were 3 cases with a density reduction of less than 2mm,and the other group had 1 case.Both groups meet the standard of bone ingrowth at the end of the follow-up period.3 hips after surgery occurd thigh pain in the Corail group,and the pain rate was 8.33%.However,the Tri-lock BPS group was not complained of thigh pain in the follow-up time.The pain rate of the two groups was statistically significant(P<0.05).Conclusion:With patients whose femoral cavity morphology belonged to Dorr A or B type,Tri-lock BPS and Corail stem had no significant difference in early hip function improvement,operation time,intraoperative bleeding,postoperative drainage and early follow-up X-ray.Tri-lock BPS has the advantage of low early postoperative pain rate,relatively high bone mass retention in surgery and smaller surgical trauma.The long-term clinical effects of two groups should be further studied.
