Objective:To measure the volumetric changes of grey and white matters in patientswithtemporal lobe epilepsy(TLE)using VBM-DARTEL and correlate the changes with executive function.Methods:16 diagnosed left-sided Temporal Lobe Epilepsy(lTLE)patients,26 diagnosed right-sided Temporal Lobe Epilepsy(rTLE)patients and 38 healthy controls(HCs)underwent routine MRI scanning and acquisition 3D fast gradient echo T1 weighted image;Use voxel-based morphometry-diffeomorphic anatomical registration through exponentiallie algebra(VBM-DARTEL)analysis method calculated grey matter and white matter volumes, and obtained statistical differences between brain regions(P<0.001,uncorrected,K = 50);the application of attention network test were used to evaluation the executive function;finally, investigate the relationship of grey and white matter volumes and executive function of patients. Results:TLE patients had diffuse cerebral atrophy, grey matter atrophy of brain regions were mainly in temporal lobes and frontal lobes. The white matters were mainly in temporal lobes and hippocampus;the right anterior lobe of greymatter in rTLE patients was less than lTLE patients;no grey and white matter volumes increased;compared to healthy controls, the TLE patients' executive network efficiency values were significantly increased(P<0.05),executive network efficiency values of the lTLE patients were less than rTLE patients(P<0.05);executive network efficiency values were negatively correlated with grey matter volumes in rTLE patients. Conclusion:There are changes in brain structure of the temporal lobe epilepsy patients. There were differences in the structure and function of patients with different origin lesions. In right-sided Temporal Lobe Epilepsy patients, the decrease in the volume of grey matters have led to a decline in executive function.
