Objective:Voxel-based morphometry based on Diffeomorphic Anatomical Registration Through Exponentiated Lie Algebra(DARTEL) was used to examine the volume changes of grey matter(GM) of patients with early and mid stage primary open-angle glaucoma(POAG) and to explore the characters of GM changes and Central nervous system injury in early- mid stage POAG and provide a new thinking of preventing POAG.Methods: Thirteen POAG patients(according to the diagnose of POAG) and Thirteen gengder-age-matched healthy control were enrolled in the study. For each participant high-resolution structural brain image was acquired on a 3.0 scanner. Grey matter white matter and csf image were obtained after preprocessing by DPARSF based on SPM. GM volume and GM%(GM/(GM+WM+CSF)) were estimated in two-sample test using spss 13,0. REST、SPM were used to analyze the change of whole brain GM and local region(limbic system、midbrain and brodman 17、18、19)Result:1)whole brain volume analysis:the gap in GM and GM% between POAG and control group was not statistically significant(GM:T=1.87 P=0.07>0.05 GM%:T=0.23 P=0.82>0.05). 2) whole brain analysis: compared with control group,volume in midbrain、brainstem thalamus 、caudate nucleus、 praecuneus、 parietal lobe、occipital lobe and vermis was increased in POAG. volume reduction could be found in cerebellum. 3)local region analysis: volume of thalamus 、caudate nucleus、midbrain and brodman18 was increased in POAG comparing with control group. 4)correlation analysis:RNFL was shown negative correlation with white matter,including occipital lobe、precuneus and brain stem(R<-0.80 p< 0.05), and positive correlation with Brodmann 18 area(R>0.80 p< 0.05). negative correlation was found between MD and occipital, frontal gyrus, thalamus and midbrain(R<-0.90 p< 0.05). It has positive correlation between MD and Brodmann 18 area(R>0.90 p< 0.05).Conclusion:Early- mid stage POAG could cause widely GM volume increasing includingvolume of thalamus 、caudate nucleus、midbrain、brainstem and brodman18 in brain. volume reduction was not obvious. VBM-DARTEL could detect minor differences in brain tissue comparing with traditional MRI. It provide a new thinking and method to explore the pathogenesis and preventing of POAG.
