Objectives:In comparison with traditional multiport laparoscopic splenectomy,to discuss the safety and feasibility of transumbilical single-incision laparoscopic splenectomy with conventional laparoscopic instruments,and summarize the clinical application experience of transumbilical single-incision laparoscopic splenectomy with conventional laparoscopic instruments,in order to provide relevant clinical data for the promotion and application of this technique in splenic surgury.Method:The clinical data of 18 patients(experimental group)who underwent transumbilical single-incision laparoscopic splenectomy with conventionallaparoscopic instruments and clinical data of 43 patients(control group)who underwent laparoscopic splenectomy with traditional five-hole method from December 2010 to August 2017,were retrospectively analyzed.The operation time,intraoperative blood loss,the incidence of complications,postoperative hospital stay,hospitalization expenses,postoperative pain scores,and the score of postoperative cosmetic result were compared and analyzed.Result:Both groups underwent successful operation.All the patients were discharged after the operation,and no serious perioperative complications were occurred.The operation time of experimental group was longer than that of control group,the difference was statistically significant(P <0.05).There was no significant difference between the two groups in the amount of bleeding,hospitalization expenses and the incidence of postoperative complications(P>0.05).The pain score of the experimental group on the first day after operation was lower than that of the control group,the postoperative hospital stay in the experimental group was shorter than that of the control group,and the score of postoperative cosmetic result was better in the experimental group than in the control group(P <0.05).Conclusion:It is safe and feasible to carry out transumbilical single-incision laparoscopic splenectomy with conventional laparoscopic instruments for those units with some experience laparoscopic surgeon,especially for non-obesepatients with normal or mild spleen enlargement.Transumbilical single-incision laparoscopic splenectomy is less traumatic than traditional multiport laparoscopic splenectomy,which has better cosmetic effect,can further reduce postoperative pain and shorten postoperative hospital stay,and has broad application prospects.
