Objective:The study is a randomized controlled clinical trial based on the result, which is obtained from the experiment examined through antifungal activity of Xuan Pao FangⅡin vitro. And it is a clinical comparative analysis of the curative effects of Xuan Pao FangⅡand Naftifine Hydrochloride and Ketoconazole Cream in treating keratotic tinea hands or pedis. The purpose of this study is to verify the validity and security of the treatment of Xuan Pao FangⅡ,and provides evidences for development and promotion of traditional Chinese medicine for the treatment of tinea pedis and tinea pedis.Methods:79 patients corresponding to the diagnosis standards of keratotic tinea hand or pedis, who came from the Department of Dermatology of the Affiliated Hospital of Shanxi University of Traditional Chinese medicine between November 1,2015 and November 31,2016,were randomly allocated into control of Western medicine group(40cases)and treatment of Xuan Pao FangⅡgroup(39cases).The treatment of Xuan Pao FangⅡgroup adopted the traditional Chinese medicine liquid which was prepared by soaking the herbal pack containing Xuan Pao FangⅡin the 3000ml white vinegar by two days.Patients soaked their feet or hands into the medicine liquid for 30 minutes after washing clear at nigh.Changed the new medicine liquid everyweek. The control of Western medicine group externally used Naftifine Hydrochloride and Ketoconazole Cream twice a day for one week(morning and evening).A course of treatment needed 4weeks.Scored the skin condition(area, hyperkeratosis, chap and desquamation)and subjective symptoms(itch and pain)of two groups of patients. All items were recorded before and after treatment and calculation of total points.Caculated the curative effect index, effective rate and fungal clearance rate of the two groups after treatment. The control of Western medicine group and the treatment of Xuan Pao FangⅡgroup were tested for safety index and recorded their values.Data were analyzed by SPSS13.0 statistical software, measurement data using t-test,count data using X~2 test, data which did not normal distribution by nonparametric test(Wilcoxon signed rank test,Wilcoxon):difference was statistically significant(P<0.05).Results:After 4 weeks of treatment, the significant efficiency and efficiency of the treatment of Xuan Pao FangⅡgroup(39 cases)were 29.1%and 20.3%;the positive rates of mycological culture and fungus microscope examination were 10.1%and 2.5%.The significant efficiency and efficiency of the control of Western medicine group(40cases)were 35.4%and 15.2%;the positive rates of mycological culture and fungus microscope examination were 7.6%and 3.8%.As a result, the effective rate and fungal clearance rate of two groups after treatment had no significant difference through statistical analysis of test between before and after treatment. The security indexes before and after the treatment did not change obviously, and did not appear adverse events in the process of study. Conclusion:The result of antifungal activity in vitro shows that Xuan Pao FangⅡreally has the antiseptic bacteriostasis effect. The clinical and experimental researches reveals that Xuan Pao FangⅡcan improve the clinical symptoms of keratotic tinea hands or pedis obviously, relieve itching, pain and other clinical symptoms quickly. The study shows that Xuan Pao FangⅡis a kind of economical and practical Chinese medical external treatment, its clinical curative effect and sterilization bacteriostasis effect are obvious, and no evidence shows the toxic effect. There was no significant effect on the treatment of other diseases. It should be a valuable option and be used widely in clinic.
