Objective To evaluate the Alvarado score for the diagnosis of acute appendicitis(AA)in children by the Meta-Analysis.Methods By Retrieving literatures from the NCBI PubMed,Embase,Wan fang,Wiper,CNKI databases during 1997-2017,We gained 19 literatures related to Alvarado score in children with acute appendicitis,and estimated all articles according to the table of Quality Assessment of Diagnostic Accuracy Studies with 11 standards?Finally,5548 children from 13 different countries and regions were acceptable to my study.Statistical software,Stata12.0 and Review manager 5.3,were used for the 21 initial data statistical analysis.The sensitivity,specificity,positive likelihood ratio and negative likelihood ratio of Alvarado score in the diagnosis of early acute appendicitis in children were calculated.Results The results of the Alvarado score for the diagnosis of acute appendicitis in children as follow: Thesensibilitywas0.83(95 % CI,0.77-0.89),the specificity was 0.80(95 %CI,0.75-0.84),the area under the curve below diagnosis is: 0.88(95%CI,0.84-0.90).The difference of race and sample size has an important influence on the outcome of the study,among which the former has a greater weight.However,neither the threshold of the Alvarado score was 6 nor 7,the diagnostic results were not significantly affected.Different years of publications do not affect the diagnosis results.Conclusion The sensitivity and specificity of Alvarado score diagnostic criteria were Stronger.The diagnosis of early AA in children could be considered by Alvarado score.Based on Alvarado score,AA was divided into high risk group,middle risk group and low risk group,it could be a guideline for children hospitalized,weather he need a emergency surgical treatment or not.
