| ObjectiveTo understand the current status of family functions in patients with moderate or severe hand injury and to analyze the factors that affect family function in patients with moderate or severe hand injury.So as to provide reference for the next step in the development of nursing intervention programs to improve family function.MethodsThe hospitalized patients with moderate or severe hand injuries who were treated at the Second People’s Hospital of Changshu from January 2017 to August 2017 were selected as subjects.General information questionnaires(including general demographic data and disease-related data)and family functions were used.Family Assessment Device(FAD),Self-Rating Depression Scale(SDS),and Self-Rating Anxiety Scale(SAS).All data were recorded into the database using Epidata 3.50 software,and SPSS 23.0 statistical software was used for statistical analysis.Measurement data were expressed asx± s,count data were expressed as frequency and percentage;single factor analysis was analyzed using t-test and analysis of variance,and multivariate analysis was analyzed by multiple stepwise regression analysis,and pearson correlation analysis,P<0.05,and the difference was statistically significant.Results(1)Family function: Among 392 patients with moderate or severe hand injury,145(37.0%)had good family function,245(62.5%)were normal,and 2(0.5%)were poor.The average value of FAD score was(126.78±19.85)points,the problem-solving average was(12.03±2.28)points,the communication average value was(17.76±3.38)points,the role average value was(25.18±4.41)points,the emotional reaction average value was(12.61±4.41)points,the emotional intervention average value was(13.41±2.76)points,and the behavioral control average value was(23.09±4.12)points,the total function average value was(22.70±4.11)points.(2)Single factor analysis of family function in hand-wounded patients: education background,marital status,place of residence,occupation,monthly income,medical payment method,family role,cause of injury,perceived pain,follow-up work,follow-up economic sources,SAS is the total score of family function And the influencing factors of each dimension;age is the total factor of family function and other dimensions except emotional involvement;follow-up compensation,SDS is the total score of family function and other factors in addition to behavior control factors;injury severity It is the total factor of family function and the influencing factors of problem solving and role dimension;gender is the influencing factor of the dimension of role and affective reaction.(3)Multivariate analysis of family function in hand-wounded patients: The education background,the total score of family function in SAS in moderate or heavy hand injuries,and the seven dimensions were statistically significant;the total score of family functions in patients with moderate or severe hand injury in the medical payment method.In addition to the behavioral control,the other six dimensions were statistically significant;the perceived total scores of family function in patients with moderate to severe hand injury and the other six dimensions except emotional intervention were statistically significant;living in The total scores of family functions and the four dimensions of severe hand injury patients were statistically significant;age was statistically significant in the dimensions of role,affective response,and behavior control;and the total score of family function in marital status in moderately severe hand injury patients.The dimensions of emotional involvement were statistically significant;SDS was statistically significant in the dimensions of emotional response and emotional intervention;and the follow-up economic source was statistically significant in the dimension of problem solving.Conclusion(1)The level of family function in patients with moderate or severe hand injuries is general;(2)Age,education background,marital status,place of residence,medical payment methods,perceived pain,follow-up economic sources,anxiety and depression are the main factors affecting the family function of moderate to severe hand injuries.(3)In terms of ages,the younger the patients,the better the family function;In terms of the education background,the higher the patients,the better the family function;when it comes to the marital status,the unmarried are superior to the married,then the widowedand last the divorced;As for residence,patients in cities are prior to those in rural areas in terms of the family function.The family functions of TCM insurance,work injury and other payment methods(including the parties and the boss payment)of the medical payment methods were better than those of the NCMS and self-paying families.The family function of patients at their own expense was the worst;Patients with self-conscious pain had worse family function than patients with unconscious pain;Patients with no follow-up economic sources have poor family function compared with patients with subsequent economic sources;the higher scores of SAS and SDS in patients with moderate to severe hand injuries were,the worse the family function was.(4)In order to improve the family function level of patients with moderate or severe hand injuries,the nursing staff should be led by family intervention and call for the attention of the whole society to promote the physical and psychological recovery of patients and return to social life as soon as possible. |
