Objective:To explore the effect of meditation training on sleep quality in elderly patients with hypertension in community.Methods:Included 2 parts to finish this subject study.In the 1st part is the survey section.A survey of the quality of sleep in 310 elderly hypertensive patients in the community to understand the sleep conditions of elderly hypertensive patients in this community by using the Pittsburgh Sleep Quality Index Scale(PSQI).The second part is intervention research.From the first part of the screening criteria of 58 cases were randomly divided into control group and intervention group,29 cases in each group.The control group took routine weekly telephone follow-up/home visit/outpatient follow-up to carry out health education on subjects and asked about general conditions such as blood pressure control,sleep quality,diet and exercise for 8 weeks.Besides the routine sleep health education and follow-up,the intervention group was dealt with the meditation training.Intervention group on the basis of routine care with meditation training,a total of 8 weeks intervention.Meditation training is divided into two phases:the first phase(the first week)for group training,carried out at the community health service center,once a day focused training,each 20min for 1 week.The second stage(the second week to the eighth week)for family practice.The subjects practiced at home and required each practitioner to perform a meditation exercise at home once a day for 20minutes.To ensure good compliance,the research team distributed a card portable speaker and a meditation management log for each subject.Also,during this phase,organize group training once a week to inquire about their feelings and compliance.Pre-intervention and intervention for 1 month,intervention for 2 months,followed up for 1 month were used the Hamilton Anxiety Scale(HAMA)\Beliefs and attitudes about sleep scale(DBAS)\Pittsburgh sleep quality index(PSQI)to evaluate the effect before and after intervention subsequently.The data were analyzed by SPSS 22.0 statistical software.The measurement data were analyzed by t test.The count data were analyzed by?~2 test.The repeated measurement data were analyzed by repeated measures analysis of variance.The rank data were analyzed by rank sum test.P<0.05 was considered as statistically significant.Results:(1)A total of 310 questionnaires were distributed in the survey,310were recovered,the recovery efficiency of 100%.A total of 176 patients(56.78%)in 310 elderly patients with hypertension(age range 65-87 years,mean 73.07±5.83 years)with poor sleep quality(PSQI>7)were included.(2)Among the 310 subjects surveyed,there were 118 cases of anxiety for38.06%(118/310),of which 109 cases had poor sleep quality,accounting for61.93%(109/176)of the total number of poor sleep quality.(3)HAMA score and factor scores:repeated measures ANOVA results showed that the intervention group HAMA total score and psychotic anxiety factors decreased compared with the pre-intervention,intervention at 1 month,intervention at 2months the difference was statistically significant(p<0.05).(4)Comparison of DBAS score and each dimension:Repeated measures ANOVA showed that the scores of DBAS and scores in intervention group were significantly higher than those before intervention(p<0.05).(5)PSQI score and each dimension score comparison:Repeated measures ANOVA results showed that the PSQI score and each dimension score(except for hypnotic drug score)in the intervention group showed a downward trend compared with the pre-intervention,with statistical significance(p<0.05).And the difference was the most significant when intervention was 2 months.Mainly reflected in the PSQI dimension mainly in improving the quality of sleep,sleep time,sleep efficiency in three dimensions.Conclusion:(1)There are sleep quality problems in elderly patients with hypertension in the community.(2)2 Meditation training can improve mental anxiety.(3)Meditation training can improve sleep beliefs,improve sleep quality,sleep time,sleep efficiency.
