BACKGOUND:The discoid lateral meniscus(DLM)is caused by abnormality of meniscus morphology.The shape of the discoid meniscus is different,but compared with the normal meniscus,it usually covers the area of the tibial plateau,and the shape is similar to that of the disc.The etiology of discoid meniscus is not fully understood,and many hypotheses have been put forward,including: congenital stagnation,acquired,tibial condylar shape determinism,etc.But the consensus is that it has to do with a number of factors.In clinical practice,the discoid meniscus is divided into three types: complete,incomplete,Wrisberg ligaments according to Wantanabe classification method.Different types of patients may have different treatment methods.Patients generally do not have obvious clinical symptoms,only when the activity of the knee joint feel uncomfortable.If there is a meniscal tear or unstable,symptoms usually appear in the lateral knee joint line bulge performance repeatedly play sound accompanied by pain,effusion,limited range of motion,and a pressure point,locking and other symptoms.If not timely treatment may accelerate the knee joint degeneration or even led to osteoarthritis,the consequence is very serious.Currently,clinical diagnosis of discoid meniscus relies on careful medical history,physical examination,imaging and even arthroscopy.Magnetic Resource Imaging(MRI)is the most effective Imaging method for examining discoid meniscus.With the increasing awareness of the disease,the diagnosis and treatment of the disease are getting more and more attention.Therefore,this paper confirmed completely,incomplete discoid meniscus in patients under arthroscopy with preoperative MRI images is studied,in order to obtain complete,incomplete discoid meniscus MRI on the relationship between the measured parameters,the same which in turn to provide the basis for accurate diagnosis and treatment,better serve for the clinical practice.OBJECTIVE: To compare the MR imaging differences between complete and incomplete discoid meniscus in adults,and establish evaluating index of different types of discoid meniscus imaging.METHODS: A total of 40 cases of complete discoid meniscus,30 cases of incomplete discoid meniscus and 40 cases of non-discoid meniscus,who were diagnosed by arthroscopic surgery in the same period,were included in this study.The Carestream Vue PACS workstation Functool software was used to measure the morphological index of cases on the knee MRI coronal imaging.The comparison of three groups was performed with the data of single factor analysis of variance(ANOVA).RESULTS: The comparison of these parameters,including fibular height(FH),the lateral meniscus width(LW),height(LH),the lateral femoral condyle cartilage thickness(LCCT)and width of the lateral meniscus / the width of tibial plateau(R1),was showed to the statistical difference(p < 0.05)among three groups.However,the comparison of those parameters,such as the tibia plateau width(TW),medial meniscus width(MW),height(MH),the medial femoral condyle cartilage thickness(MCCT)and width of the medial meniscus body/the width of tibial plateau(R2),had no statistical difference(p > 0.05)among them.According to the method of LSD on the three groups,the positive measured values were analysed furtherly in each group.It was found that the differences of FH,LW and R1 were statistically significant(p < 0.05)in them while the LH was showed no statistical differences(p >0.05)between incomplete type group and non-discoid meniscus group.In addition,the value of LCCT had no statistical differences(p >0.05)between complete type and incomplete discoid meniscus.These results indicated that there was a significant difference in morphological changes in MRI on complete discoid meniscus,incomplete discoid meniscus and non-discoid meniscus.CONCLUSION:The value of FH,LW and R1 in MRI on discoid meniscus may be reliable morphological parameters for the assessment of complete discoid meniscus and incomplete discoid meniscus in adults.
