[Background]The incidence of colorectal cancer ranks third in the world, and the incidence of colorectal cancer ranks fourth in China. There are about 250,000 new cases every year, accounting for 8.3%of all new tumors,of which about 147,000 are men.10.7 million people died from about 140,000 cases of colorectal cancer each year.Liver metastasis is an important factor influencing the prognosis of colorectal cancer and is the main cause of increased mortality.About 30%to 50%of colorectal cancer patients cannot avoid the outcome of liver metastasis.Current treatment options for colorectal liver metastases include liver metastases surgery, systemic chemotherapy, targeted therapy, transcatheter arterial chemoembolization, radiofrequency ablation, and radiotherapy.Surgical resection is the main method to treat liver metastasis of colorectal cancer. The median survival time of metastases cannot be removed is 6.9 months, and patients with complete liver resection can reach 35 months, but due to the size of liver metastases Whether the site, liver metastases,or physical conditions are tolerant to surgery, psychological factors, treatment costs, and other factors, most patients cannot undergo surgical treatment. However, the current clinical efficacy is not ideal for patients with liver metastases that cannot be operated. In recent years, the clinical efficacy of Chinese medicine for cancer patients has received attention. The pathogenesis of colorectal cancer liver metastases is mostly" Benxubiaoshi","Pixu" is the main factor, with "Tan","Re","Yu","and" Du".Jianpifa has an irreplaceable role in the clinical treatment of tumors in traditional Chinese medicine.A number of studies have found that single-taste Chinese medicines such as ginseng and astragalus that have the effect of JianpiYiqi can regulate autoimmune and induce tumor cell apoptosis, regulate the role of tumor microenvironment.According to clinical findings, traditional Chinese medicine Jianpifa can significantly improve the clinical symptoms of patients with colorectal liver metastases, assist in preoperative preparation, promote the recovery of postoperative functions, reduce adverse reactions to postoperative chemotherapy, improve the quality of life of patients, and make patients have a confidence in treatment and prolong the survival.[Aims]By comparing the traditional Chinese and Western medicine-based regimen based on Jianpifa with the Western-only treatment regimen,we explored clinical treatment approaches for colorectal cancer with liver metastases.[Methods]60 patients with colorectal liver metastases were divided into experimental group and control group each with 30 cases.Control group:Primary surgery + postoperative systemic venous chemotherapy mFOLFOX6 regimen+ Lentinan Infusion;Experimental group:Primary surgery+postoperative systemic venous chemotherapy mFOLFOX6 regimen+"Jianpifang"-based Chinese herbal decoction orally. All patients are followed up for 1 year. From the aspects of changes in the number of CD3+CD4+CD8+T-lymphocyte subsets, serum CEA.CA199 levels,Response Evaluation Criteria in Solid Tumors(recistl.1),Improvement of clinical symptoms and side effects of chemotherapy,KPS scores, and Progression-free survival(PFS),a comprehensive analysis of the clinical efficacy of the two groups is conducted.[Results]After treatment, the number of CD3+,CD4+,and CD8+ cells in the experimental group was significantly higher than that in the control group(P<0.05),but there was no significant difference between the control group and the control group(P>0.05).;the CR+PR+SD rate in the experimental group was higher than that in the control group(86.67%&56.67%),and the difference was statistically significant(P<0.05);The clinical symptoms and side effects of chemotherapy in the experimental group were significantly higher than those in the control group. The difference between the two groups was statistically significant(P<0.05);the KPS score in the experimental group was better than that in the control group, and there was a statistically significant difference between the two groups(P<0.05);The 18-month survival rate was 63.33%in the experimental group and 40%in the control group. The median progression-free survival time was 13.5 months and 8.5 months respectively.Compared with the control group, the median survival time of the PFS increased by 7.0 months in the experimental group.[Conclusion]Compared with western medicine alone, integrated Chinese and Western medicine based on spleen-enhancing therapy can improve the immune function of patients with liver metastasis of colon cancer, prolong the patients' non-progressive survival, reduce the toxic side effects of chemotherapy, and improve the quality of life of patients. Therefore, traditional Chinese medicine treatment based on spleen-enhancing method is a good supplement for patients with liver metastasis of colorectal cancer that is unresectable for metastasis.
