| ObjectiveExplore the response to therapy and prognosis of genetic immunology for MDS under the intervention of traditional Chinese combine western medicine providing the basis for combination of tiered treatment of MDS.MethodsUsing retrospective observational research methods, collect between December 2009 and September 2015 for the first time in Beijing university of Chinese medicine during the east onset ward or clinic included in the standard of 38 patients with MDS, analyzes their general clinical features and clinical curative effect, and explore the correlation between the YiSui Paste combined western medicine therapy of MDS patients in clinical efficient, the improvement of peripheral blood cell count, the median survival time and leukemia conversion rate with cytogenetic, cellular immunology and molecular biology.Results1, The clinical curative effect, the clinical total effective rate is more than 60%. The clinical efficient of MDS patients with better karyotype after treatment is higher than MDS patients with poor karyotype, but no significant difference statistically (P> 0.05);The clinical efficient of MDS patients with lower FCSS integral after treatment is higher than MDS patients with higher karyotype, especially the clinical efficient of patients with MDS who’s FCSS integral is in 0-1 count is more than 80%, but no significant difference statistically (P> 0.05); The clinical efficient of MDS patients with gene mutations are not found is higher than MDS patients with gene mutations, but no significant difference statistically (P> 0.05).2, The improvement of peripheral blood cell count, each karyotype group of MDS patients’s before and after treatment of the WBC, NEUT, HGB, PLT count has no obvious difference (P> 0.05);the WBC count of MDS patients who’s FCSS integral≧ 4 increased significantly after treatment, he difference was statistically significant (P< 0.05),the other each FCSS group of MDS patients’s before and after treatment of the WBC, NEUT, HGB, PLT count has no obvious difference (P> 0.05);Gene mutation of MDS patients’s before and after treatment of the WBC, NEUT, HGB, PLT count has no obvious difference (P> 0.05).3, The median survival time, The median survival time of MDS patients with better karyotype after treatment is higher than MDS patients with poor karyotype, but no significant difference statistically (P> 0.05); The median survival time of MDS patients with lower FCSS integral after treatment is higher than MDS patients with higher FCSS integral, the difference was statistically significant (P< 0.05); The median survival time of MDS patients with gene mutations are not found is higher than MDS patients with gene mutations, but no significant difference statistically (P> 0.05).4, Leukemia conversion rate, the leukemia conversion rate of MDS patients with poor karyotype after treatment is higher than MDS patients with better karyotype, there was no statistically significant difference (P> 0.05); the leukemia conversion rate of MDS patients with higher FCSS integral after treatment is higher than MDS patients with lower FCSS integral, the difference was statistically significant (P< 0.05);The leukemia conversion rate of MDS patients with gene mutations are not found is higher than MDS patients with gene mutations, but no significant difference statistically (P> 0.05).Conclusion1, Karyotype of bad group shows on the low side in the field of clinical effectiveness, as though statistical analysis with the other two groups have no obvious difference, may be affected by sample data is too small lead to deviation statistical data, but the early stage of the combined with our department to summarize the efficacy of MDS analysis of IPSS prognostic scoring system, the clinical effectiveness of bad karyotype group using EPO combines traditional Chinese medicine treatment is poor. It should be treated with stem cell transplant or to methylation such treatment.2, FCSS integral has influence on the median survival time of MDS patients after treatment, and FCSS integral is negatively correlated with the median survival time; FCSS integral has influence on the leukemia conversion rate of MDS patients after treatment, and FCSS integral is positively correlated with the median survival time.So MDS patients whose FCSS integral≥4 should also be used as soon as possible treatment such as stem cell transplantation or treatment to methylation.3, According to this study, it is unable to determine if there is correlation between the genetic mutations or not and traditional Chinese combined western medicine treatment of MDS in clinical curative effect, the improvement of peripheral blood cell count,the median survival time, and any related aspects such as leukemia conversion rule. |
