Objective:To investigate the clinical diagnosis,differential diagnosis and treatment of patients with femoral vein compression from synovial cyst of the hip joint.Methods:A retrospective analysis of 21 patients diagnosed as hip synovial cyst compression femoral vein from January 2008 to February 2018 in the First Affiliated Hospital of Zhengzhou University(21 cases with lower extremity swelling,3 cases of lower extremity venous insufficiency,deep vein of lower extremity Thrombosis in2 cases).21 patients underwent Color doppler ultrasound,16 patients underwent computed tomography(CT)or magnetic resonance imaging(MRI)postoperative diagnosis and treatment(with lower extremity venous insufficiency in patients with simultaneous high saphenous vein ligation and varicose veins,Accompanied by deep venous thrombosis of lower extremity,simultaneous CDT treatment or femoral vein thrombectomy).Five patients underwent color Doppler guided percutaneous puncture after MRI or CT diagnosis.Postoperative tissue was sent for routine pathological examination.Results:A total of 21 patients with hip synovial cysts were diagnosed,with anaverage age of 48.5 years.Thirteen women(61.9%)and eight men(38.1%)were women.All patients showed unilateral lower limb swelling.Five patients(23.8%)had palpable inguinal masses,three(14.3%)had venous insufficiency,and two(9.5%)had venous thrombosis.Five patients underwent percutaneous puncture drainage guided by color Doppler ultrasound.Two of the patients had cyst recurrence one month and three months after operation.The remaining 16 patients underwent surgical resection and no recurrence of cysts was observed during the current follow-up(mean,22.7 months,median follow-up,17 months).The pathological results indicated that synovial cysts were considered.Conclusions:Color doppler ultrasound screening can be used as the first choice of diagnosis of the disease.MRI or CT can provide a good guide for surgical treatment.Puncture treatment can effectively relieve the clinical symptoms of patients,but the recurrence rate of cysts is higher than that of surgical treatment.Surgical treatment of synovial cysts can be used as the preferred method of oppression of the femoral vein,postoperative symptoms improved significantly,the recurrence rate of cysts low.
