Objective:The current development of domestic transitional care has just started and lacks mature standards and models.Based on the comprehensive domestic and international data,we conducted a study on the continuing care of children with chronic diseases,constructed a transitional care model for primary nephrotic syndrome in children,and provided continuous nursing services for children with primary nephrotic syndrome.The purpose is to promote its rehabilitation,improve its quality of life,and at:the same time provide the basis for the development and implementation of transitional care for all types of children with chronic diseases.Methods:1.Based on the current development status of domestic and foreign transitional care,relevant national documents,and experts' opinions in related fields,the research team has initially established the basic framework of the continuation care model for children with primary nephrotic syndrome.2.After two rounds of Delphi enquiries,41 experts identified the various items and model frameworks,and established a transitional care model for the children with primary nephropathy.SPSS software was used to statistically analyze the indicators of the mean and coefficient of variation of expert opinions.Results:1.The preliminary draft we designed includes 4 first-level entries,20 second-level entries,23 third-level entries,and 36 fourth-level entries,which is the first round of questionnaires of the expert consultation.2.Through two rounds of expert correspondence,the opinions of experts were unified.The transitional care model for children with primary nephropathy was established.It includes four major parts:the staff constitution of transitional caremodel for children with primary nephrotic syndrome,the responsibility of staffs of the transitional care model,the work content of the transitional care model,the effect evaluation index of the transitional care model.The content includes 4 first-level entries,20 second-level entries,23 third-level entries,and 36 fourth-level entries.After analyzing the weights of the items at all levels,a transitional care model for children with primary nephrotic syndrome was constructed.3.Delphi expert consultation reliability analysis:the letter experts must fully understand the purpose and significance of this study,and then evaluate the content of the study.It is generally believed that when the authority coefficient score(Cr)is>0.7,the authority of experts is high.The judgment coefficient of this research experts is 0.84,the familiarity coefficient is 0.76,the authoritative coefficient is 0.80.The authoritative coefficient is greater than 0.7,indicating that the degree of authority of the expert is high,which provides a strong guarantee for the reliability of the results of this research.The coefficient of variation is smaller,the degree of coordination among experts is higher.Generally the coefficient of variation must be less than 0.25.The coefficient of variation of the two rounds of correspondence in this study is between 0.02 and 0.23,all less than 0.25,indicating that the result of the inquiry is desirable.The coordination coefficient W is between 0 and 1.For the same group of experts and same indicators,the larger the W,the better the degree of coordination is.The coordination coefficients of the two rounds of experts' opinions in this research are 0.458 and 0.327.The significance test results show that:P<0.01,the difference is statistically significant,indicating that the expert opinions tend to be consistent.Conclusion:1.The transitional care model for children with primary nephrotic syndrome includes four components:staff constitution,responsibility of staffs,work content,and effect evaluation index of the continuation care.It can provide scientific working methods for the continuation of nursing care for children discharged from primary nephrotic syndrome.2.The letter experts in this research is representative and enthusiasm,a high degree of authority.The coordination of the two rounds of expert correspondence is good,the experts' opinions tend to be consistent.The transitional care model for children with primary nephrotic syndrome in this study is high reliability,scientific,and authoritative.3.The transitional care model for children with primary nephrotic syndrome is based on the needs of children and provides a basis for the implementation of continuous nursing care for children with chronic diseases.4.In the transitional care model for children with primary nephrotic syndrome constructed in this study,the multidisciplinary team provided the implementation method for the continuation of the nursing after discharge.This care model can ensure the continuity,professionalism,and permanence of the continuation of the nursing service,and is adapting to the development of modern medical care technologies and models,has strong feasibility.
