BackgroundInflammatory bowel diseases(IBD)is a kind of chronic inflammation of the gastrointestinal tract, including Crohn’s disease(CD)and ulcerative colitis(UC).It is a life-long idiopathic disorder and the etiology of IBD is unknown.Three major pathways likely activate the disease: a genetic predisposition, immune dysregulation, and an environmental antigen. However, the etiology of pediatric inflammatory bowel disease(PIBD)might different from adult-onset IBD,Inflammatory bowel disease occurs worldwide, but the highest incidence is found in North America, the United Kingdom, and northern Europe. The incidence of Crohn’s disease has risen slowly over time, although ulcerative colitis remains slightly more prevalent than Crohn’s disease.Symptoms of inflammatory bowel disease are varied and lack of specificity,to identify this disease we need combine the clinical characteristics, endoscopic evaluation and laboratory findings. The precisely diagnose of the disease is essential for the treatment and prognosis. However, there are limited studies of the PIBD,let alone the study of PIBD in Asia.Because of the limitation of studies, there have not been a guide to index the diagnosis and treatment,so it is necessary to add more information to PIBD.Therefore, individualized treatment plans should be formulated.ObjectTo compare the clinical characteristics of pediatric inflammatory bowel disease(PIBD)in the First Affiliated Hospital of Zhengzhou University,Zhengzhou China with Korean and European, which from the 5-year European multicenter study of children with new onset IBD(EUROKIDS registry). To achieve the correct diagnosis and appropriate therapy.MethodsChildren who were diagnosed with IBD between May 2011 and October 2016 were investigated at the First Affiliated Hospital of Zhengzhou University.Their clinical characteristics were retrospectively evaluated by medical record review. And the results were compared with Korean and European.Results1.A total of 21 children with Crohn’s disease(CD)and 32 children with ulcerative colitis(UC)were enrolled,33.3% pediatric-onset Crohn’s disease with whole digestive tract imaging examination,90.5% with colonoscopy,5.9% ulcerative colitis with whole digestive tract imaging examination,94.1% with colonoscopy.2.In comparison with the Korean and European, there is no gender difference.3.In comparison with EUROKIDS,Chinese pediatric CD had a higher prevalence of terminal ileal disease(38.1%vs16.3% P=0.018),and a lower prevalence of ileocolonic disease(14.3%vs52.8% P=0.001),and concomitant/isolated upper gastrointestinal tract(UGI)disease(14.3%vs46.2% P=0.003).For disease behavior,Chinese pediatric CD had a higher prevalence of stricturing(33.3%vs13.2% P =0.018),and Chinese pediatric CD had a lower prevalence of nonstricturing,nonpenetrating(61.9%vs82.0% P=0.04).There was no difference of perianal disease between Chinese and European. In comparison with EUROKIDS,Chinese pediatric UC had a higher prevalence of ulcerative proctitis(15.6%vs4.7% P=0.004).4.Except for a lower prevalence of ileocolonic disease(14.3%vs50.0% P=0.016),there was no difference between Chinese and Korean.ConcludesThe results suggest that characteristics of Chinese pediatric IBD patients and European pediatric IBD patients may be different. And there are no many differences between Chinese and Korean on pediatric IBD patients.
