The Diagnostic Value Of Dynamic Ultrasound Combined With Lidocaine Injection In Subacromial Impingement Syndrome

Objective: To evaluate the clinical value of dynamic ultrasound combined with lidocaine injection in the diagnosis of subacromial impingement syndrome.Method:The study enrolled 34 patients suspected of subacromial impingement syndrome through the way of dynamic ultrasound with 7-10 MHz high frequency linear array probe combined with lidocaine injection by the same doctor and magnetic resonance imaging(MRI)and finally accepted arthroscopy from June 2016 to December 2017.There were 21 males and 13 females,Aged 32-77 years(mean,23.6 years).Right shoulder was noted in 26 patients and left shoulder in 8 patients,11 patients had a history of obvious injury.Compared to the intraoperative findings of shoulder arthroscopy,and then calculated the sensitivity,specificity,positive predictive value,negative predictive value,accuracy and Youden index of the two methods for the diagnosis of subacromial impingement syndrome.Result: As confirmed by arthroscopy,dynamic ultrasound combined with lidocaine injection group and MRI group detected 20 and 19 out of 22 subacromial impingement syndrome cases respectively.The sensitivity,specificity,positive predictive value,negative predictive value,accuracy and Youden index of dynamic ultrasound combined with lidocaine injection group was 90.9%?83.3%?90.9%?83.3%?88.2% and 74.2% respectively,and that of MRI group was 86.4%?66.7%?82.6%?72.7%?79.4% and 53.1% respectively for assessment of subacromial impingement syndrome.Except for Youden index(P<0.05),there was no significant difference in the two groups(P>0.05).Conclusion:1.Dynamic ultrasound combined lidocaine injection is a new,safe and effective method of inspection.It has the characteristics of convenient operation,low prices,real-time dynamic observation,which is suitable for popularization and application in primary hospitals.2.Dynamic ultrasound combined with lidocaine injection and MRI have similar high sensitivity,specificity and accuracy for detecting subacromial impingement syndrome.3.Dynamic ultrasound combined with lidocaine injection perform an ideal method for subacromial impingement syndrome assessment which can be used as the first choice of examination before shoulder arthroscopy...
