Objective:Establishment of a duodenal jejunal bypass animal model that retains different lengths of Roux intestinal fistulas in diabetic rats(GK rats)?By analyzing the changes of preoperative and postoperative fasting blood glucose,Hb A1 c,C-peptide,and insulin levels in five groups of diabetic rats,To investigate the effect of different Roux intestinal fistulas on glucose metabolism in type 2 diabetic rats,and to explore the optimal length of Roux intestinal fistula.Methods:In the formal experimental part,a total of 50 male GK rats of similar age and age were selected and randomly divided into 5 groups of 10 rats each;A group: control group;B group: Roux intestinal fistula 10 cm group;C group: Roux intestinal fistula 20 cm group;D group: Roux intestinal fistula 30 cm group;E group: Roux intestinal fistula 40 cm group.Five groups of GK rat orbital venous blood were collected preoperatively and at 1,3,6 and 12 weeks after surgery.Using electronic scales and blood glucose meters to dynamically measure the weight of each rat and the level of blood fasting blood glucose(FPG).The fasting insulin(FINS)and glycosylated hemoglobin(Hb A1c)levels were measured before and after surgery in each rat using enzyme-linked immunosorbent assay(ELISA),to compare the effect of different Roux intestinal fistulas on glucose metabolism in GK rats.Results:During the entire observation period from 1 week to 12 weeks after surgery,there was no significant change in the body weight of GK rats in each group.Compared with fasting blood glucose levels in preoperative and postoperative time points in group A(control group),There was no statistically significant difference(P>0.05).The fasting blood glucose levels of GK rats in groups B,C and D decreased significantly with time,and the difference was statistically significant(P<0.001).At each postoperative time,the fasting blood glucose level of GK rats decreased significantly with the length of Roux intestinal fistula,and the difference was statistically significant.There was no significant difference in fasting insulin values between preoperative and postoperative time points in group A(control group)(P = 0.07).The fasting insulin values of GK rats in groups B,C and D decreased significantly with time,and the difference was statistically significant(P<0.05).At each postoperative time,the fasting insulin value of GK rats decreased significantly with the length of Roux intestinal fistula,and the difference was statistically significant(P<0.05).There was no significant difference in fasting C-peptide between preoperative and postoperative time in group A(control group)(P=0.27).The fasting C-peptide values of GK rats in groups B,C,and D decreased with time,and the difference was statistically significant(P<0.05).At each postoperative time,the fasting C-peptide value of GK rats decreased significantly with the length of Roux intestinal fistula,and the difference was statistically significant(P<0.05).During the whole observation period from 1 week to 12 weeks after surgery,there was no significant difference in fasting Hb A1 c values between preoperative and postoperative time points in group A(control group)(P=0.73).There was no significant difference in fasting Hb A1 c values between preoperative and postoperative 1 week of GK rats in groups B,C,D,and E(P>0.05).From 1 week to 12 weeks postoperatively,fasting Hb A1 c values in groups B,C and D showed a decreasing trend with time,and the difference was statistically significant(P<0.01).At each time period,the fasting Hb A1 c value of GK rats decreased with the length of Roux intestinal fistula,and the difference was statistically significant(P<0.05).Conclusion:1.In this study,based on non-obese type 2 diabetic rats(GK rats),a duodenum jejunal bypass animal model of different Roux bowel loops was successfully established,and its effect on glucose metabolism in GK rats was explored.2.DJB surgery has a significant therapeutic effect on non-obese type 2 diabetic rats,and can effectively improve the glucose metabolism of GK rats.3.The treatment effect of GK rats in the Roux gut 30 cm group was the best,and the length of the Roux gut was likely to lead to dysfunction of GK rats,causing malnutrition and other metabolic complications,and even led to death in rats.
