Echocardiographic Evaluation Of The Effects Of Peritoneal Dialysis On Left Ventricular Structure And Function In Uremic Patients

Objective: Application of conventional two-dimensional echocardiography and real-time full volume three-dimensional echocardiography in the study of left ventricular structure,function,and left ventricular 17 th myocardial systolic and diastolic synchrony before and after peritoneal dialysis in uremic patients,and explore the effect of peritoneal dialysis on left ventricular shape and function in uremic patients significance.Methods: 52 cases of uremic patients(case group)who were scheduled to undergo maintenance peritoneal dialysis(MPD)in the Department of Nephrology in our hospital and 40 cases of healthy checkers(control group)were selected.The left lateral decubitus position was examined and the chest lead electrocardiogram was connected.Using siemens acuson sc2000 color doppler ultrasound diagnostic apparatus,4V1 c 2D heart probe and 4Zlc full volume cardiac probe were selected to measure left ventricular structural volume parameters: LAAPD,IVSEDD,LVPWEDD,LVEDD,LVESD,2D-EF,3D-EF,LVEDV,LVESV;left ventricular synchronization parameters: SDI17,DDI17,DISPES17,DISPED17,ESSI,EDSI,MES17,MED17,Pre Contr17,Post Contr17,Pre Relax17,Post Relax17,etc.,echocardiography after MPD treatment,and with Comparisons were made before treatment and in the control group.Results: Comparison of left ventricular structural volume parameters: Before treatment,LAAPD,LVEDD,LVESD,LVEDV,LVESV,and LVMI were increased in the case group compared with the control group.IVSEDD and LVPWEDD were thicker than those in the control group(P<0.05);Six months after MPD,LAAPD,LVEDD,LVESD,LVEDV,and LVESV decreased compared with those before dialysis(P<0.05).IVSEDD,LVPWEDD,and LVMI decreased compared with those before dialysis,and the difference was not statistically significant.Six months after MPD,LAAPD,LVEDD,LVESD,LVEDV,LVESV,LVMI were increased compared with the control group.IVSEDD and LVPWEDD were thicker than those in the control group(P<0.05).Comparison of left ventricular synchronous parameters: Before treatment,SDI17,DDI17,ESSI,ESDI,DISPES17,DISPED17,Pre Contr17,Post Contr17,Pre Relax17,and Post Relax17 in the case group were greater than those in the control group(P<0.05).);MES17,MED17 compared with the control group was not statistically significant;Six months after MPD,SDI17,DDI17,ESSI,ESDI,DISPES17,DISPED17,Pre Contr17,Post Contr17,Pre Relax17,Post Relax17 decreased compared with before dialysis,the difference was statistically significant(P<0.05);Six months after MPD,SDI17,DDI17,ESSI,ESDI,DISPES17,DISPED17,Pre Contr17,Post Contr17,Pre Relax17,and Post Relax17 were increased compared to the control group,and the difference was statistically significant(P<0.05).Conclusion: The uremic patients had thicker left ventricular wall thickness before and after dialysis than the control group.The left ventricular mass index increased compared with the control group,the left ventricular volume increased compared with the control group,and the left ventricular dyssynchrony was more disordered than the control group;the peritoneum of the uremic patients.Six months after dialysis treatment,left ventricular volume decreased compared with that before dialysis,left ventricular synchronization was improved to some extent;real-time whole-volume echocardiography can quantitatively evaluate the effect of maintenance peritoneal dialysis treatment on left ventricular structure and function in uremic patients.Diagnosis and treatment have important guiding significance.
