Background and Objective:Alopecia areata(AA)is an inflammatory and non-scarring hair loss disease.It is often recured in most patients,and delayed for years and decades.Hair loss is severely impacted patients' psychology,work and social activities.It is a long history that Chinese Traditional Medicine had realized the pathogenesis and treatment of AA.The formation of blood stasis played a minor or major role in the cause of AA,and promoting blood circulation to remove blood stasis method is a good therapeutic principle for treating AA.However,there have been few studies for its mechanism at present.Currently,It is generally accepted that AA is an organ-specific autoimmune disease mediated by T cells.It has been revealed that the T cells and cytokines regulated the cause and development of AA,and interferon-gamma played an important role in the pathogenesis of AA.The present research of modern theory of stasis blood syndrome have showed that oxygen free radical damage was the main mechanism of blood stasis syndrome,characterized by the changes of SOD and MDA.In this subject,fasting IFN-gamma,SOD and MDA level of peripheral blood serum were detected in AA patients with blood stasis syndrome and non-blood stasis syndrome AA patients.Then their corelation were analyzed to explore an immunological mechanism of AA caused by blood stasis.It provided a new view to interprete blood stasis type AA in immunological perspective.Method:80 patients with a clinic diganosis of AA and 20 healthy volunteers were included in the study.Fasting IFN-gamma,SOD and MDA level in peripheral blood serum were detected.Then the difference of IFN-gamma,SOD and MDA level were compared each other according to the blood stasis syndrome differentiation standards,different severities of illness,different course and illness activity.And the corelation between IFN-gamma and SOD,between IFN-gamma and MDA were analyzed in blood stasis syndrome and non-blood stasis syndrome AA patientsResults:There was no statistical difference of gender,course of the disease,illness severity and disease activity between Blood stasis type AA and non-Blood stasis type AA.IFN-gamma level in heavy alopecia areata group was higher than light spot bald group(P<0.05),which was also higher than normal control group(P<0.01).IFN-gamma level in activity alopecia areata group and recovery alopecia areata group were higher than normal control group,the difference was statistically significant(P<0.01),but activity period alopecia areata group and recovery alopecia areata group had no significant difference.IFN-gamma level in course>1 year group and course ? 1 year group were higher than normal control group(P<0.01),the difference was statistically significant(P<0.01),but course>1 year group and ourse? 1 year group had no significant difference.IFN-gamma level in Blood stasis type AA group were higher than non-Blood stasis type AA(P<0.01),which was higher than ormal control group(P<0.01).SOD level in heavy alopecia areata was lower than light spot bald group(P<0.01),which was also lower than normal control group(P<0.01).SOD level in activity alopecia areata group and recovery alopecia areata group were lower than normal control group,the difference was statistically significant(P<0.01),but activity period alopecia areata group and recovery alopecia areata group had no significant difference.SOD level in course>1 year group and course group were lower than course ? 1 year group(P<0.01),that were lower than normal control group(P<0.05).SOD level in Blood stasis type AA group were lower than non-Blood stasis type AA(P<0.01),which was lower than normal control group,(P<0.01).MDA level in heavy alopecia areata group was higher than light spot bald group(P<0.01),which was also higher than normal control group(P<0.01).MDA level in activity alopecia areata group and recovery alopecia areata group were higher than normal control group,the difference was statistically significant(P<0.01),but activity period alopecia areata group and recovery alopecia areata group had no significant difference.MDA level in course>1 year group and course group were higher than course-1 year group(P<0.01),that were lower than normal control group(P<0.01).MDA level in Blood stasis type AA group were higher than non-Blood stasis type AA(P<0.01),which was higher than normal control group(P<0.01).IFN-gamma changes were positively correlated with MDA level and and negatively correlated with SOD activity change in blood stasis type AA.But these corelation was not found in non-Blood stasis type AA.Conclusion:IFN-gamma is an important factor for the pathogenesis of alopecia areata,and is associated with disease severity and TCM syndrome types.SOD and MDA was also associated with the onset of alopecia areata,corelating with disease severity and course,especially in the blood stasis syndrome.And IFN-gamma level changes associated with the degree of oxygen free radical damage.As a result,we speculated that status of oxidation and antioxidant and abnormal expression of IFN-gamma was a significant mechanism for blood stasis to cause AA,and they regulate each other.
