Day surgery can greatly speed up the turnover rate of hospital beds,reducing medical pressure and improving patient satisfaction.It is one of the key tasks of the reform of health system proposed by the General Office of the State Council.Surgical scheduling is the key to control the efficiency of day surgery.The previous study is mainly focus on ordinary surgery,that each department makes schedule separately.Therefore,the process is relatively simple and the differences are always not considered.At the same time,the resource categories are fixed at each stage,lacking in flexibility.The article firstly introduces day surgery,and proposes a lean day surgical process suitable for China's conditions.It analyzes the current scheduling method and explains the problems and improving space of the current method.Next,in view of the characteristics of the unified treatment of day surgery,the paper considers the impact of different anesthesia types on resource occupation and the concept of occupying the previous operating room or resuscitation bed according to the situation during resuscitation,establishing a scheduling model for deterministic time.According to the model,the corresponding genetic algorithm is designed.Compared with the Cplex and the resource-fixed model,it is proved to be more effective by giving real examples.The resource allocation and the relationship between the scheduling indicators are also analyzed.In order to facilitate the application of the model,a program is designed according to the realistic scene of a hospital in Shanghai,and the interface,function and operation method are introduced.On this basis,the robust optimization method is chosen to build the model for uncertain time to reduce the disturbance caused by time fluctuations.According to the model,the combination optimization algorithm is designed.The genetic algorithm is used as the outer layer to determine the operation allocation and order,and the internal point method is used as the inner layer to determine the fluctuation of the operation time.The goal is to optimize the objective under the worst case of fluctuation.Comparing the robust model with the deterministic model and the current method,it shows better effect and better robustness.Model exploration,configuration optimization and index analysis is provided to make it easier to apply.Finally,based on lean thinking,a two-stage scheduling method is proposed to improve the turnover rate of the beds.Surgeries that does not need to be observed is put first,and surgeries that need to be observed are in back row,an example is also given.
