In order to ensure the utilization of basic medical services for urban workers,reduce the economic burden caused by diseases and improve the health level of workers,Urban workers' basic medical insurance has been implemented since 1998.The establishment of basic medical insurance system for urban workers has raised the level of medical health and promoted the further development of economy and society.The appropriate level of medical insurance payment is the foundation of the sustainable development of medical insurance system in China,which is of great significance for guaranteeing the basic medical rights and interests of insured employees and reducing the burden of insured workers.At present,the academia has not yet formed a consensus on the basis of medical insurance payment level,reference indicators,moderation judgment and other issues.How to ensure both the insured workers' medical and medicine have been reported,and ensure the payment subject affordability moderate,has become an important issue facing the development of the basic medical insurance for urban workers.This paper changes the traditional research ideas of the Urban workers' basic medical insurance optimum payment rate,bases on the goal of optimal,attempts to analyse it from the point of enterprises and workers,then put forward the concept of“optimum payment rate”.Among them,within the relevant laws and regulations of the state,the enterprise payment rate refers to the proportion of the basic medical insurance paid by the enterprise to the total amount of the workers' wages.The individual payment rate of workers refers to the proportion of the basic medical insurance premiums paid by individual workers to the individual wage income.Under the condition of other factors,the level of enterprises payment rate and workers payment rate determine the level of medical insurance.Whether the payment rate of the enterprise is optimum or not will directly affect the profitability and market competitiveness;whether the workers payment rate is moderate or not will directly affect the current consumption level and the expectation of the future consumption.Therefore,determining the optimum payment rate has important practical significance.The paper is divided into five parts to discuss:The first part is introduction.Pointing out the Research Purposes and Research Significance?Overseas and Domestic Research Status?Research Ideas and ResearchMethods as well as Innovation and Shortcomings.The second part is Related Conception and Theory Summary.Defining The extension of Urban workers' basic medical insurance?Premium rate and Optimum level.Four theories have been proposed around the topic,including Equity and Efficiency,Cost-Sharing of public goods,The social reproduction theory of Marx as well as Life-Cycle and Consumption-Savings.The third part is The Evaluation of Urban Workers' Basic Medical Insurance Premium Rate.This part systematically combs the History and Current situation of Urban Workers' Basic Medical Insurance premium rate in Zhejiang,and there exists the following problems:High premium rate burden on enterprises,a serious shortage of finance subsidies.then,analyzing the influence of the related problems.The forth part is Empirical Research.Applying production function and consumption function,we calculate enterprises premium rate and workers' individual premium rate respectively,the results show that:(a)In recent years,the gap between overall optimum payment level and statutory payment level is small,basically fitting.This shows that the overall payment level is reasonable,which is in line with current economic levels.(b)The legal level of enterprise payment rate is 8%,while calculated optimum premium rate is much lower than 8%,which shows that the enterprise payment burden is heavy and is not conducive to the development of enterprises;secondly,the overall payment ability of enterprises in northeast is better than those in southwest Zhejiang;(c)The workers statutory payment rate is 2%,while the calculated optimum premium rate is much higher than 2%,which indicates that the basic medical insurance payment of individual workers can withstand 2% rate,workers payment ability is largely underestimated.The fifth part is Conclusions and Policy Recommendations.Conclusions include the following:Reducing enterprises premium rate?Increase workers' individual premium rate and Increase government subsidies.Policy Recommendations are as follows:Delay retirement age?Investment management of balance funds,Establish a legal system of Urban Workers' basic Medical Insurance,perfect workers' individual payment way,make clear the government's financial subsidy reponsibility and establish financial responsibility growth mechanism consistent with economic growth.
