The Research Of Basic Medical Insurance System Integration For Urban And Rural Residents In Jiujiang City

With the continuous development of the economy and the society,a universal medical insurance system which covers all Chinese people,from the enterprises & the institutions,the flexible employment group of privately or individually-owned business or others,to the majority of urban and rural residents,has been gradually established in China.The system has been designed to achieve full coverage.The objective of the medical reform,which is “Health Insurance for All”,has realized partially.However,the three different types of medical insurance systems,including urban workers' medical insurance system,the new rural cooperative medical insurance system and the urban residents' medical insurance system,are operated fragmentarily and separately.It is to the disadvantage of the continuous development of the systems,and is not conducive to the principle of “Equality for All” advocated by China.With the accelerating integration process of the urban and rural areas and the flow of people across regions,the voice of masses for equal access to medical care is growing.Currently,the implementation of the household registration system reform,the dual endowment insurance structure of urban and rural residents or other reforms which breaks urban and rural dual structure have created favourable conditions for the reform for dual medical insurance of urban and rural residents.In the January of 2016,the State Council announced formally,that it required to establish a unified medical insurance system which covers both the national wide urban and rural residents.This is a signal that we have entered into the full implementation stage of the integration of urban and rural medical system.In the beginning of the thesis,based on the status quo of domestic and foreign research,the author introduces basic definition of the urban and rural resident's integration,the theory of dual economic structure and the theory of public goods.Then,the author applies approaches including comparative analysis,descriptive analysis and questionnaires to introduce the operations of urban residents' medical insurance system and the new rural cooperative medical insurance system in recent years in Jiujiang.Through more than 10 years of development,the target of the urban and rural medical insurance which is “insurance covers all,system secured all,and positive feedback received from the insured people” has been achieved.Meanwhile,several problems exist,for instance,the dual structure is separately managed,residents duplicateregistered,discrepancy of compensation level exists and the medical resources are imbalancedly allocated.Finally,based on summarizing the successful experiences of the pilot cities in eastern China and middle-west China,the author analyses the favourable conditions for the integration of medical insurance systems of urban and rural residents,and summarized the measures to implement the integration of medical insurance system of urban and rural residents in Jiujiang City,including improving the system,integrating the agencies and constructing the IT system etc.The author draws the preliminary conclusion to the integration of the essential medical insurance system of urban and rural residents in Jiujiang City.In the current state,the externally political,economic and social conditions and internal system structure is in readiness for the integration of urban residents' medical insurance system and the new rural cooperative medical insurance system in Jiujiang.To establish an integrated medical insurance system for urban and rural residents,we should actively and reliably carry forward the integration project for urban residents' medical insurance system and the new rural cooperative medical insurance system in a staged approach,in conjunction with the status quo of the system implementation in Jiujiang.
